About this file

This is a Proposer Information Form for RFP #13-13-26 issued by the City of Temple, Texas for Vending Machine Services. The RFP seeks to establish an annual services agreement for the supply, installation, and servicing of vending machines at designated municipal facilities. The form requires prospective vendors to provide company information, acknowledge receipt of any addenda issued through the e-procurement portal, authorize credit checks to verify financial responsibility, identify authorized personnel, confirm ability to execute the attached Purchase Agreement and comply with insurance requirements, and state any exceptions to RFP terms. Vendors must submit proposals in accordance with the RFP timeline and submission instructions outlined in the document.

The form indicates that successful proposers must be able to execute a service agreement substantially as presented with no substantive changes allowed, and must obtain required insurance coverage within five days of notice of intent to award. The awarded vendor is responsible for all damage to people and property resulting from performance of contracted work and will be held liable for errors or failures. The City reserves the right to terminate agreements for unsatisfactory performance, including excessive service callbacks to correct the same problem within 30 days, lack of vendor personnel expertise, or failure to provide timely service. Invoices shall be submitted monthly in arrears itemized by service location and date, emailed to invoice@templetx.gov, and must reference the City's purchase order number. The City retains the right to add, modify, or delete services or property locations through contract amendments.
