This is an instructional guide for filling out an III Log form for base access at Andersen Air Force Base. The document provides detailed formatting requirements for completing the III Log, which requires only three mandatory fields: Subject (Last Name, First Name in alphabetical order), Date of Birth (in YYYY-MM-DD format), and Social Security Number (full number required).

The guide includes examples of correct and incorrect III Log entries, emphasizing that no additional fields should be completed beyond Subject, DOB, and SSN. It specifies that the form's format must not be altered even when submitting information for a single person, and includes contact information for the 36th Security Forces Squadron (36 SFS) Visitor Control Center at 366-5650/1/2/3 or 36sfs.vcc@us.af.mil for questions or assistance.
