About this file
This is a Past Performance Questionnaire template for solicitation number N0017426R1012, issued by the Navy (Contract Specialist: Diane Hicks). The questionnaire is designed to collect past performance information on offerors for contracts not already documented in CPARS (Contractor Performance Assessment Reporting System).
The questionnaire is divided into two completion sections: offerors may complete only the "Contract Information" portion (offeror name, contract number, and period of performance), while the customer POC must complete all remaining sections and submit directly to the Contract Specialist on or before the proposal due date. The customer evaluation section requests information on five performance categories: customer satisfaction (responsiveness, personnel qualification, cost estimation accuracy); timeliness (task completion within requested timeframes); technical success (understanding of SOW, first-time task completion, problem resolution); program management (subcontractor management, cost/schedule/performance control); and quality (reliability of services delivered). Respondents use a five-point rating scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory) plus a Neutral option for insufficient information. The questionnaire also includes an overall rating requirement and two open-ended subjective questions regarding contractor recommendation and any special or unique problems. The Government explicitly states it will not assign rankings to the past performance factor but will determine acceptability based on provided information and other available sources.
