About this file
This is a Vendor Declaration Statement form issued by the Collier County Board of County Commissioners Procurement Services Division in Naples, Florida, for use in conjunction with the Arbitrage Rebate Services Request for Proposals. The form requires vendors submitting proposals to certify that their bid is made without collusion or fraud, in compliance with the County's Procurement Ordinance and ethical standards, and that no elected or appointed County officials or their relatives have a financial interest in the proposed services. Vendors must also declare that they have fully examined and accepted all instructions, terms, conditions, requirements, and specifications outlined in the solicitation and agree to accept a Purchase Order or execute a formal Collier County contract if awarded. The form requires vendors to disclose any related entities serving as subcontractors and prohibits markup of subcontractor fees when the subcontractor is a related entity of the vendor.
The Arbitrage Rebate Services procurement seeks qualified proposers to provide annual calculations of arbitrage rebate on the County's tax-exempt debt issuances to maintain compliance with federal tax code and support annual financial reporting requirements. Historical annual spending for these services has averaged $20,100 over the past six years, though this figure may not be indicative of future purchasing patterns. Vendors must certify under penalties of perjury that all information provided is accurate and must be signed by a duly authorized corporate representative with signature authority.
