About this file

This is a pricing schedule submission form for Request for Proposal No. 260000002166 issued by the State of Michigan for an Air Monitoring Shelter. The document establishes the pricing structure that contractors must complete and submit as part of their proposal response. Contractors are required to provide pricing for one air monitoring shelter unit, with separate line items for shipping costs and any additional expenses such as estimated tariff costs. The pricing schedule must be submitted in a modifiable format, such as Microsoft Word or Excel, with the option to submit an additional copy in non-modifiable format such as PDF. Failure to complete the pricing schedule as requested may result in proposal disqualification.

The pricing proposal must include all costs associated with the contract activities, including one-time charges, setup fees, shipping and handling, per-piece pricing, and palletizing costs. Contractors are encouraged to offer quick payment discount terms by specifying a percentage discount off the invoice if payment is received within a specified number of days after invoice receipt, excluding processing time. By submitting a proposal, the contractor certifies that all prices were developed independently without consultation or agreement with other contractors. The total cost for the air monitoring shelter, shipping, and any other expenses must be clearly calculated on the pricing schedule.
