About this file

The document is a Darfur Contracting Act Certification form from the California Department of General Services (DGS) Procurement Division, which applies to companies seeking to bid on State of California contracts. The form requires companies that have had business activities outside the United States within the previous three years to certify that they are either not a "scrutinized company" or have obtained written permission from the Department of General Services to submit a proposal. Companies with no international business activities in the past three years are exempt from completing the form.

The certification process offers two options: Option #1 allows companies to certify under penalty of perjury that they are not a scrutinized company, while Option #2 provides a pathway for scrutinized companies to obtain case-by-case permission from DGS to bid on state contracts if it is in the best interests of the state. Companies choosing Option #2 must include a copy of their written permission from DGS with their bid or proposal. The form requires the company's name, Federal ID Number, an authorized signature, and the printed name and title of the person signing the certification.
