About this file
This document is Form E of the City of Sun Prairie's Request for Proposal (RFP #25-PRF15) and establishes the insurance and indemnification requirements for contractors and consultants providing consulting, management, operations, and maintenance services. The form outlines comprehensive insurance coverage requirements that must be in place prior to commencement of work and remain in effect throughout the project duration or as specified in the contract agreement.
The insurance requirements mandate that contractors maintain Worker's Compensation and Employers Liability Insurance with limits of $1,000,000 per accident and $1,000,000 disease policy limits, Commercial General Liability Insurance with $1,000,000 per occurrence limits ($2,000,000 general aggregate if applicable), Automobile Liability Insurance with $1,000,000 per occurrence limits, Umbrella Liability Insurance with $2,000,000 per occurrence limits, and Professional Liability Insurance with $1,000,000 per claim and annual aggregate limits. All policies must be written on Insurance Services Office (ISO) standard forms or equivalent endorsements and must name the City of Sun Prairie and its officials, officers, employees, and authorized representatives as additional insureds. The contractor's insurance must be primary coverage, shall not be canceled without 60 days' prior written notice to the City (10 days for non-payment), and must be provided through carriers with minimum A.M. Best's ratings of A-VII. Prior to contract execution, contractors must submit certificates of insurance (Acord Form 25-S) along with required endorsements to the City of Sun Prairie Finance Department. Contractors employing sub-contractors must require each sub-contractor to meet the same minimum insurance requirements. The indemnification clause requires contractors to defend and hold harmless the City from all claims, damages, and legal expenses arising from the contractor's performance, except those caused solely by the City's negligence or willful misconduct, with this obligation surviving contract termination.
