About this file
The document is a Labor Code Certifications form that outlines key labor compliance requirements for a state contract. The form requires contractors to certify compliance with California Labor Code provisions, including workers' compensation insurance, prevailing wage requirements, and apprenticeship regulations. Contractors must ensure workers are paid prevailing wages, with penalties of $200 per day for each worker paid less than the stipulated rate, and must limit work hours to 8 hours per day and 40 hours per week, with a $25 penalty for violations.
The certification requires contractors and subcontractors to employ properly registered apprentices at standard wage rates for their specific craft or trade, and to maintain accurate records as specified in Labor Code §1776. Contractors must also be prepared to forfeit penalties to the state for various labor code violations, including improper wage payments and excessive working hours. The form provides a space for the contractor's signature and date, indicating their agreement to and understanding of these labor code requirements.
