Amendment 0001 Dated 5/18/2021
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested in response to this notice and a written solicitation will NOT be issued. This solicitation is being issued as a Request for Quote (RFQ). Submit only written quotes for 246-21-Q-0048 for Modular Buildings. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-05. This solicitation is a 100% SB Set-Aside. The associated NAICS code is 321992 with a small business size standard of 500 Employees.

Evaluation: FAR 52.212-2 Evaluation -- Commercial Items (completed as follows):
(a) The Government will award a firm fixed price contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

· Delivery Schedule – The contractor shall provide a delivery schedule outlining delivery within 90 days ARO. The contractors shall submit a contingency schedule showing the manufacturing and delivery process to ensure delivery within in 90 days.
· Past Performance – The contractor shall provide three reference for projects of like or similar size in dollar value that occurred within the last three years. The references need to address delivery, cooperation with clients, efforts of like or similar requirements.

This solicitation will result in a firm fixed price purchase order pursuant to the terms and conditions below. Terms and conditions other than those stated will not be accepted. The above pricing is all inclusive.

Specifications/ Amendment 0001
I. PURPOSE:
The purpose of this contract is to provide one (1) new modular clinic building to Claremore Indian Hospital. The Claremore Indian Hospital is a Federal hospital within the Indian Health Service agency of the U.S. Department of Health and Human Services.
II. BACKGROUND
A new clinical modular building shall be acquired to assist Claremore Indian Hospital in the fight against the COVID-19 pandemic. Additional interior clinic space is essential in maintaining a quality of care required for the physical, mental, social, and spiritual health of American Indians and Alaskan Natives.

The contractor shall provide weekly updates on the delivery schedule after award and provide a minimum (1) week notice before delivery to the Contracting Specialist and Contracting Officers Representative (COR).
The contractor will take care of any permitting needed to transport the modular building pieces and equipment to the site. Once on site, there is no additional permitting requirements due to the facility being Federally-owned.
The delivery shall be include installation on the site specified within the attached Site Picture and Map.
IV. MINIMUM TECHNICAL REQUIREMENTS
The modular building will have to meet or exceed the following requirements:
1. Minimum dimensions of 52 Ft x 64 Ft (WxL), max no larger than 65 Ft x 74 Ft, approximately 4 modular units wide.
a. Building main entrance for patients will face north. A side exit shall face east towards the parking lot.
b. Building shall be placed more than 10’ away from existing buildings.
2. Modular building shall be constructed to the State of Oklahoma observed building code year editions and the IHS A/E Design Guide 2019 pertaining to outpatient medical facility use:
a. National Fire Codes are used exclusively for Life Safety compliance. Where a conflict
b. National Fire Codes (with the exception of NFPA 5000 – Building Construction and Safety Code), as published by the National Fire Protection Association shall be used exclusively for determining and maintaining life safety requirements for all occupancies in the new and existing facilities. If a conflict should arise between the National Fire Codes (NFC) and any other model codes on an issue of life safety, the NFC shall take precedence.
i. Fire alarm system to be installed by others.
ii. Fire sprinkler system not required.
c. NFPA 101 Life Safety Code – new business occupancy.
d. FGI Guidelines for Design & Construction of Outpatient Facilities - 2018 edition
i. Nurse call systems are not required
ii. Medical gas systems are not required
e. International Building Code 2015 or newer
f. International Mechanical Code for (outpatient medical facilities) 2015
g. International Electrical Code 2015
h. International Plumbing Code 2015
i. Architectural Barrier Act (ABA). ADA does not apply to Federal Buildings. However, ABA references many but not all ADA requirements, so American Disabilities Act will be acceptable.
j. NFPA 70 National Electrical Code -2014
k. NFPA 99 Healthcare – 2012 as applicable to outpatient facilities

3. Building shall have:
a. Minimum of eight (8) exam rooms, eight (8) screening rooms, and one (1) procedure room. Rooms to be divided in to four (4) patient-care areas (2 exam, 2 screening) with the procedure room centrally located. The exam and screening rooms will have one staff station in each room. i. All exam, screening, and procedure rooms must meet Joint Commission standards
1. Porcelain or stainless steel sink (no storage capability underneath)
2. Room to install a small table/desk for a work surface to accommodate written or electronic documentation.
3. Upper and lower cabinets to accommodate sink, medical supply storage.
4. Space for one visitor to sit in a chair.
ii. Exam rooms will be a minimum of 100 sq ft
1. 80 sq ft being clear floor space
iii. Screening rooms shall be a minimum of 70 square feet
iv. Procedure rooms shall be a minimum of 130 square feet
b. Two small offices to locate four (4) staff each, centrally located between two patient-care areas. Staff to use modular workstations provided by others.
c. One small medicine room with upper and lower cabinetry, sink with manual-operated lever handles.
d. One (1) Janitor’s closet
e. Two (2) Storage spaces
i. One for sterile/clean storage
ii. One for general storage
f. One (1) Waiting room
g. One (1) Reception desk/area for two staffed positions adjacent to the waiting area
i. Counter with opening window to access waiting area
h. Two (2) to three (3) ADA compliant Restrooms
i. One (1) to two (2) unisex restrooms
ii. One (1) unisex restroom accessible from waiting area
i. One (1) Breakroom, can be combined with reception desk area
i. Porcelain or stainless steel sink
ii. Water access for fridge or icemaker
iii. Upper and lower cabinetry for storage
j. Two alcove areas at each entrance hallway to exam areas
i. Facilitates space for weigh scale and PPE
k. Minimum one (1) year comprehensive warranty on labor and building materials, standard manufacturer’s warranty on equipment, utilities, other as applicable.
l. Waiting area, offices, and breakrooms on exterior walls shall have a window. Rooms such as storage rooms, exam rooms, and janitor’s closets do not receive a window.
4. Flooring
a. Vinyl Composition Tile
i. 1/8” Armstrong Standard Excelon, Imperial Textile or similar. Color to be selected with other interior finishes.
b. ¾” decking with suitable material
c. R-30C Insulation
5. Interior Finishes
a. Rubber/vinyl composition Wall Base 4”, color to be select with other interior finishes.
b. Wall finishes shall be protected by wall protection (FRP, wall cladding, etc.) in all areas. No paint textured drywall allowed unless approved by COR.
c. Colors to be selected with all other interior finishes and materials with color, finish, grade options.
d. Available colors shall be aesthetically pleasing and conducive to patient-care healing environments pertaining to each area. No all-white finishes.
e. All interior finishes shall be infection-control friendly and easily cleanable.
6. Roof
a. R-38C Insulation
b. Drip Rail / J-Rail
c. Building will have guttering installed by others
d. Sloped Metal Roof. No mansford or false mansford roofs.
i. Specify slope in proposal
7. Windows
a. Vinyl – Low E – Insulated Glass Windows
b. Minimum of 24” from Doors
c. Mini Blinds for Windows
8. Doors
a. Exterior:
i. Two (2) swinging metal doors with vision kits (i.e. window to see through door)
ii. Locations: Front entrance to waiting room facing north and staff use on east side (hitch side as well) of the building.
iii. Keyed similar, prefer D1 keyed cylinders
iv. Latching exit device/panic bar
v. Hydraulic door closer
vi. ADA Compliant levers/door hardware, approaches to pull side of doors.
b. Interior:
i. Commercial-grade ADA compliant lever locksets.
ii. Hydraulic door closer where required.
iii. Provide core cylinders to facility for rekeying, prefer D1 keyed cylinders
iv. Kickplates
v. Solid core 1-3/4 constructed swinging doors.
9. Electrical and Lighting
a. Minimum 100 Amp Indoor Breaker, Max 200 Amp per modular unit
b. LED Panel Lights
i. 4000k to 5000k in patient care rooms
ii. Dimmable in procedure room.
iii. 3500k to 4000k in waiting room
c. LED Panel for Restroom
d. LED Exterior Lights with Photo Control
i. Minimum one (1) on each side
ii. Minimum two (2) on each side with doors comply with NFPA 101 lighting requirements for egress exiting (loss of one light does not leave area without illumination).
e. Occupancy Sensors for interior lighting
f. 20A 3-Way commercial-grade switches
g. Hospital-grade 20A receptacles
i. Spacing requirements per NFPA 70 and FGI Guidelines for Outpatient Facilities.
ii. One (1) hospital grade 20A receptacle in each restroom (GFCI)
iii. Tamper-resistant hospital-grade receptacles in public spaces (waiting room and hallways).
h. Four (4) exterior weather resistant 20A receptacles, one on each side of the building
i. Exhaust fan vented to outside for each restroom
j. Light switches and receptacles shall be labeled for power source location by panel and circuit breaker number. Coordinate electrical panel naming terminology with COR.
10. IT
a. Install four (4) inch junction boxes with conduit to facilitate Government install of phone and data (Voice-over-IP). Cabling to be installed by others.
b. Minimum ¾” conduit shall be installed in walls to accommodate multiple CAT 6 cabling.
i. Coordinate locations with COR for up to 34 conduit locations for up to 60 CAT 6 cable runs back to existing IT areas.
11. Plumbing
a. Water Heater – Electric (no gas or propane)
b. ADA Compliant Toilets
c. ADA Compliant Faucet and Sink w/ Mirror
d. Grab bar and toilet paper holder – ABA/ADA Compliant
e. Plumbing shall be installed on interior walls. No plumbing is to be installed in exterior walls.
f. All patient-care and staff sinks shall have manually-operated lever-style faucets with legionella-friendly aerators.
g. Public areas shall have auto-sensing faucets with adjustable temperature control.
12. Stairs and Ramps
a. Temporary stair for both doors
b. Must have 3 Ft x 3 Ft (WxL) landing on top of stairs to building
c. Permanent stairs and ramps by others
13. Air Conditioning and Heating – design for expected occupant load
a. Minimum 3-ton per modular unit or appropriate
i. Verify occupancy load for HVAC in proposal
b. Ducting and returns pre-installed (ALL DUCTED). Install chase to accommodate where required.
c. Zone dampers where applicable
d. Reheat where applicable

14. Color Selection
a. Roof and Trim – Brown or similar (match existing Administration building roof)
b. All color and material choices to be selected by IHS. Vendor to provide physical samples to COR to select from before installing.
c. All color choices for interior finishes shall be aesthetically pleasing and conducive to promoting healing in a patient-care environment.
15. Exterior Walls
a. Exterior surface shall be constructed to have EIFS installed by others after delivery and protected during delivery. No skirting or trim is required.
Site Picture and Map - Quotes must include all aspects of the delivery and installation to bring the building to a ready to use state, minus any hookups (electrical, plumbing, internet, etc) that CIH should desire. This would include delivery and install of, OSHA/ADA compliant steps, and blocking & leveling.
The design layout provided by CIH is a basis of what is desired and not a required layout. It is used as a reference and example for the types of spaces preferred.
After award, there will be a submittal listing of equipment and supplies with warranty documentation and service manuals.
V. Additional Requirements
· Full sets of prints will be included with an initial design for the proposal.
· 65%, 95%, 100% or other partial-to-full 100% prints provided to COR for review before construction commences.
· As-Built drawings shall be provided after award.
· Prints shall include detailed architectural (top and side views of exterior for example), mechanical, electrical and plumbing features such as HVAC system layout, electrical system layout, ceiling plan, plumbing system layout, etc. as applicable.
· Drawings will be stamped by a Professional Engineer for the State of Oklahoma.
· Communicate with COR on a weekly basis after NTP to maintain coordination up to delivery.
· Confirm ABA/ADA compliance of doors, hardware, etc.
· Provide site installation requirements including crane use to install building.
· List out warranties for each item as required
· Roofing warranty
· HVAC warranty
· Water heater warranty
· Etc.
