Statement of Work Installation of Security Cameras and DSX Security System
Anadarko Indian Health Clinic Anadarko, Oklahoma
A. General
1. The intent and objective of this Statement of Work is to install Security Cameras and expand the DSX system in the Anadarko Indian Health Clinic’s PHN and Wellness buildings.
2. Work shall be done while the Clinic is occupied and operational. Work may be required to be performed during and after normal working hours. Normal working hours are Monday-Friday from 8:00 a.m. to 5:00 p.m.
3. Contractor shall abide by all Safety and Infection Control Policies and Procedures of the Lawton Service Unit.
4. The Anadarko Indian Health Clinic is a tobacco free campus. No smoking is permitted in the hospital campus grounds.
5. A site visit is highly recommended to determine exact locations and measurements for the installation of security cameras.
6. All contract vendor personnel are to sign in with the Security Officer prior to access of job site and notify Safety Officer/Facilities Maintenance at the beginning of each work day. At the end of each work day each vendor personnel are to sign out with the Security Officer.
B. Reference Documentation
1. Refer to attached documentation for approximate locations of DSX equipment/Cameras. Reference drawings include arrows for the cameras to denote the direction the cameras are facing.
C. Services to be Performed by the Contractor
1. Contractor shall provide all labor, materials, supplies, tools and equipment, transportation, supervision and all other necessary items to complete the tasks listed below. All installed systems must interconn
a. Installation of Cameras in PHN Building
i. The Contractor shall supply and install 2 – external IP 360 omni lens security cameras/night vision equal or comparable to (Omin IP Cameras model AV20175DN-28), 2 – internal IP security cameras equal or comparable to a (Axis M30 Network Camera Series).
1. Refer to attached drawing for camera locations/orientations in PHN building
2. All IP security cameras will be PoE interfaced
ii. Contractor must meet the following criteria for the Security Cameras:
1. PTZ IP Corner Mount Cameras: 12 Megapixel interior/exterior Omni-directional, wall mount bracket with junction box (7 cameras)
2. Mount caps:
i. 2 - 6mm lens (exterior cameras)
ii. 2 - 8mm lens (exterior cameras)
iii. 2 - 12mm lens (exterior cameras)
iv. 2 - 16mm lens (exterior cameras)
3. Axis are 4 mp/backslash vandal proof camera mount brackets.
i. 2 - mp/backslash vandal proof camera mount brackets (interior cameras)
ii. 2 – mound brackets (interior cameras)
iii. Cameras must be interfaced with the NVR in the IT server room network at the Lawton Indian hospital.
1. Contractor shall test and ensure the security cameras are fully functional with the existing security camera system
i. Cameras will interface with the Lawton Indian Hospital’s security camera’s software
2. Contractor shall provide (2) external cameras designed to meet outside seasonal weather conditions (rain, snow, sleet, heat, cold, etc.)
3. Contractor shall provide a professional installation of external and internal cameras.
i. External will be installed and weather proofed with caulking, no wiring shall be exposed, trim shall be provided around the base of the camera brackets, no holes shall be exposed, shall be watered tight sealed
ii. Internal cameras will be installed with no wiring exposed, trim shall be provided around the base of the camera brackets, no holes shall be exposed
4. CAT-6 cable is provided and installed by the owner.
i. CAT-6 cables is already positioned for camera placement
iv. Contractor shall pull any needed cabling through conduit to the Lawton Indian Hospital’s server room and ensure cameras interface with system.
b. Installation of Cameras in Wellness Building
i. The Contractor shall supply and install 3 – external IP 360 omni lens security cameras/night vision equal or comparable to (Omin IP Cameras model AV20175DN-28), 3 – internal IP security cameras equal or comparable to a (Axis M30 Network Camera Series).

c. Installation of DSX security system in PHN Building
i. DSX Security System (Contractor shall provide all DSX equipment per specifications) Refer to Attached drawing and DSX specifications.
1. DSX server will be installed within the IT closet in Wellness Building.
2. DSX devices will be located at doors specified within Attached drawing.
3. DSX server will be connected to network within the IT closet.
4. DSX devices will be powered according to attached specifications.
5. DSX devices installed will be compatible with doors within the facility.
6. DSX door access readers must be compatible with PIV cards and Proximity Key Fobs
7. Electrical door strikes must be compatible with building door frames and existing DSX infrastructure.
ii. Installation of the DSX system:
1. Install 3 – external HID multi-class card readers per the attached drawing, items will be terminated on the DSX panel located in the IT room.
2. Install 1 – internal HID multi-class card readers per the attached drawing, items will be terminated on the DSX panel located in the IT room.
3. Install 1 – surface mount strike locks per the attached drawing, items will be terminated on the DSX panel located in the IT room.
a. The 1 internal door will require a yale lock set and installation per the attached drawings.
4. Install 1 – DSX panel in the IT room with power supply.

i. DSX Security System (Contractor Shall provide all DSX equipment per specifications)

2. DSX server will be installed within the IT closet in Wellness Building.

4. DSX server will be connected to network within the IT closet.
5. DSX devices will be powered according to attached specifications.
6. DSX devices installed will be compatible with doors within the facility.
7. DSX door access readers must be compatible with PIV cards and Proximity Key Fobs
8. Electrical door strikes must be compatible with building door frames and existing DSX infrastructure.
ii. Installation of the DSX system:
1. Install 4 – external HID multi-class card readers per the attached drawing, items will be terminated on the DSX panel located in the IT room.
2. Install 2 – internal HID multi-class card readers per the attached drawing, items will be terminated on the DSX panel located in the IT room.
3. Install 2 – push bar electric strikers per the attached drawing, items will be terminated on the DSX panel located in the IT room.
4. Install 5 – surface mount strike locks per the attached drawing, items will be terminated on the DSX panel located in the IT room.
a. The 2 internal doors will require yale lock sets and installation per the attached drawings.
5. Install 1 – DSX panel in the IT room with power supply.
a. Install 1 – LAN Module in the IT room with power supply.
6. Per the attached drawing, items will be terminated on the DSX panel located in the IT room.
Testing of System/ Warranty/Training
i. The Contractor shall test all installed equipment to ensure that all equipment is functioning properly.
ii. The Contractor shall address any noted issues through the use of a punch list prepared with IHS.
iii. Warranty
1. Contractor shall provide a warranty for a minimum of 1 year after final acceptance of security cameras
iv. Contractor Shall provide training to Security and IT staff
v. Contractor shall provide 3 copies of manuals for any installed equipment.
e. Final inspection
i. Contractor shall perform a final inspection with the Security Supervisor, IT Supervisor, and Contracting Officer.
