About this file
This document is an Application Response Form for a Request for Response (RFR) issued by the Commonwealth of Massachusetts Executive Office of Health and Human Services Department of Public Health (DPH) for the Public Health Care Services Capacity Enhancement program. The RFR seeks applications from licensed hospitals, clinics, community health centers, pharmacies, provider organizations, and other healthcare organizations to provide therapeutic services, serve as regional leads for therapeutic services, deploy medical teams, and manage and coordinate healthcare, medical, and ancillary support services. Applicants can apply for one or more of the four service areas outlined in the RFR. The RFR provides detailed instructions for completing the application form, which requires applicants to describe their relevant experience, capacity, and any start-up costs associated with the services.
The application form also includes sections for applicants to provide their maximum rates for the highest-paid professionals involved in the projects, affirm compliance with various requirements, and submit the completed application through the COMMBUYS online procurement system. The RFR is part of a state and local contract opportunity in Massachusetts, with a contract term and any renewal options to be determined upon engagement with selected applicants.
