About this file

This is a Construction Contract Project Manual containing General Terms and Conditions for a paving project issued by the Florida Department of Military Affairs for the 4700 Area Paving project at Camp Blanding Joint Training Center in Starke, Florida. The work consists of repaving four streets (Sandford, Woonsocket, New Haven, and Bonifay Streets), replacing culverts with like-for-like installations where new paving crosses them, adding concrete mitered ends, and removing grass from roadway edges and driveways. The contract is classified as Level 4 or Level 5 based on contract value ($200,000–$500,000 for Level 4; over $500,000 for Level 5). The project must achieve substantial completion within 90 calendar days after the Notice to Proceed. A mandatory site visit and pre-bid meeting are required for all bidders. The document was revised in June 2023 with supplementary terms issued July 22, 2026.

Key contractual requirements include liquidated damages assessed at $177.00 per calendar day if substantial completion is not achieved by the deadline. All bidders exceeding $100,000 must submit a bid bond. The Contractor is responsible for providing and maintaining a 12' x 12' watertight office for the Architect/Engineer, installing a telephone with the Contractor bearing all charges except long distance, furnishing all water and electricity for construction and testing, erecting a project sign per specifications, and maintaining 24-hour site security from Notice to Proceed through Substantial Completion. Criminal history background checks are required for all employees and agents working on Department of Military Affairs facilities. The Contractor must comply with Davis-Bacon prevailing wage requirements if applicable. All costs associated with insurance, bonds, permits, connections, and services are the Contractor's responsibility.
