About this file

This is a Schedule B Pricing form for Request for Proposal No. 220000002425 issued by the State of Michigan for IT Asset Disposal Services. The document establishes the pricing structure that contractors must complete and submit as part of their proposal. Contractors are required to provide per-piece pricing for secured items including computers, laptops, servers, output devices, flat panel monitors, media disposal, smartphones, tablets, and iPads. For non-secured items such as scanners, CD-ROM burners, disk arrays, server racking, monitors, and cords, pricing is requested on a per-skid basis, with each skid defined as a 4' x 4' pallet of equipment stacked approximately 2' to 4' high.

Pricing proposals must be submitted in a modifiable format such as Microsoft Word or Excel, with an optional non-modifiable PDF version permitted as a supplement. All costs must be included in the pricing schedule, encompassing one-time or setup charges, fees, shipping and handling, per-piece pricing, and palletizing charges. Contractors are encouraged to offer quick payment discount terms, with the structure allowing a percentage discount off the invoice if payment is received within a specified number of days after invoice receipt, excluding processing time. Contractors certifying their proposal must attest that prices were developed independently without consultation or agreement with other contractors. Failure to complete the pricing schedule as requested may result in proposal disqualification.
