About this file

This document is an evaluation worksheet for a custodial services contract, likely for a state or local government facility, specifically focused on a comprehensive performance assessment for a service provider. The worksheet details a rigorous monthly, quarterly, and annual evaluation process for custodial services, with specific performance criteria covering daily, weekly, monthly, and annual cleaning and maintenance tasks. The evaluation covers multiple aspects of service delivery, including common area cleaning, restroom sanitation, trash removal, floor maintenance, dusting, window cleaning, and specialized tasks like HVAC grill cleaning and carpet steam cleaning.

The evaluation matrix is designed to systematically track provider performance across numerous metrics, including communication responsiveness, invoice accuracy, equipment readiness, safety compliance, and adherence to specific cleaning protocols. The document includes detailed checklists for tracking service performance, with specific requirements for EPA-approved cleaning procedures, COVID-19 related sanitization, equipment standards (such as HEPA filter vacuums and commercial-grade floor buffers), and periodic deep cleaning tasks. The worksheet provides a comprehensive mechanism for monitoring and rating the service provider's performance, with options to document compliance, identify deficiencies, and potentially impact contract continuation based on the evaluation results.
