About this file
This is a Qualification Form issued by the Minnesota Department of Natural Resources for a state and local contract opportunity. The document outlines specific qualification requirements for potential contractors, including two primary criteria: (1) the responding company must have completed at least one prime contract with a value of $10,000 or more since September 2023 with a similar scope of work, and (2) the company must not have been found in default or had a contract terminated for cause in the last 36 months. Contractors are required to provide detailed information about previous project references, including property owner details, contract value, and substantial completion dates.
The qualification form serves as a screening mechanism for the state to evaluate contractor responsibility, with the state reserving the right to contact references and review various aspects of a contractor's performance. Specifically, the state will consider factors such as project milestones, communication with project owners and subcontractors, on-site supervision, coordination of subcontractors, compliance with Minnesota prevailing wage laws, and payment practices to subcontractors. Any affirmative response to having been found in default or having a contract terminated will result in automatic rejection of the solicitation response.
