About this file

This document is a certification form for the Darfur Contracting Act, which applies to companies bidding on or submitting proposals for State of California contracts and have had business activities or operations outside of the United States within the previous three years. The form requires the company to certify that it is either not a scrutinized company or has received written permission from the Department of General Services to submit a bid or proposal. The certification must be signed by an authorized representative of the company, and the Federal ID number must be provided.

The document outlines two options for completing the certification. Option 1 is for companies that are not scrutinized, and Option 2 is for companies that are scrutinized but have received written permission from the Department of General Services to submit a bid or proposal. The form also includes instructions for completing the ADA form.
