About this file
This is a Request for Qualifications (RFQu) issued by the City of Eugene, Oregon, Finance Division/Purchasing Office, seeking to establish a qualified pool of veterinary medical service providers for spay/neuter surgery and well-pet care services. The City of Eugene's Spay & Neuter Clinic currently operates surgical services four days per week, performing approximately 2 canine ovariohysterectomies, 2 canine orchidectomies, 5 feline ovariohysterectomies, and 4 feline orchidectomies during 3 to 4-hour windows, along with a daily wellness clinic providing vaccinations, parasite control, and in-house rapid blood tests for 25-30 patients plus 20-25 additional in-person clients requiring counter services. Qualifications must be submitted by November 1, 2025, with statements accepted on a rolling basis. Inclusion in the qualified pool does not guarantee contract awards; rather, the City will review pool participants' qualifications when specific service needs arise and may enter into contracts with appropriate providers. Contracts issued to pool participants may vary in size with a not-to-exceed limit of $100,000 per fiscal year.
Minimum requirements for pool acceptance include current Oregon veterinary licensure and demonstrated ability to perform surgical sterilizations, administer and monitor anesthetics, conduct pre- and post-operative examinations, inoculate animals, supervise post-operative care, implant microchips, record controlled substances, and maintain organizational and medical protocols while communicating effectively with clinic staff. All applicants must submit a Statement of Qualifications, completed questionnaire, resume, professional references form, Independent Contractor Certification Statement, and Standard Qualification Form through OregonBuys or directly to the Purchasing Analyst. Applicants are classified as independent contractors responsible for their own state and federal taxes and are not considered City employees. Professional liability insurance with limits of not less than $1,000,000 is required, and contractors must maintain coverage for at least one year after work completion. The contract includes standard City of Eugene provisions addressing fair employment practices, tax law compliance, non-discrimination, record maintenance, and dispute resolution, with exclusive jurisdiction in Lane County Circuit Court or the District of Oregon.

1.5 Inclusion of Fair Employment Practices Provisions in Contracts with Subcontractors. The contractor shall include the provisions of subsections 1.1 through 1.4 above in contracts with subcontractors so that the provisions will be binding upon each subcontractor.
