About this file
This document is a Vendor Questionnaire for a Dump Truck Rental with Driver Request for Bid (RFB) issued by Sacramento County Department of Transportation for their Maintenance and Operations Section. The solicitation seeks to establish multi-year, annual contracts for dump truck rental services, with an initial one-year contract period estimated to begin June 1, 2024, and potential renewals for up to four additional one-year periods, totaling a possible five-year contract term. The County reserves the right to award multiple contracts and allocate work as deemed most beneficial, with no guaranteed minimum work volume. The RFB requires vendors to provide detailed information about their organization, including founding date, history, equipment inventory, driver experience, safety practices, emergency contact information, and fleet size.
The questionnaire specifically requests vendors to detail their fleet composition, including quantities of 3-axle end dump, 5-axle transfer, and 5-axle bottom dump trucks. Vendors must demonstrate operational capabilities by providing information about their equipment, driver qualifications, subcontracting relationships, and emergency response procedures. Additional requirements include submitting a current business license, motor carrier permit number, and identifying vendor representatives for equipment requests and after-hours communication. The comprehensive questionnaire aims to assess vendor capability, safety standards, and operational readiness to support Sacramento County's transportation maintenance and operations needs.
