About this file

This document is the 2018 Annual Self-Certification form for the New Jersey Behavioral Risk Factor Surveillance System (BRFSS). The BRFSS is a collaborative project between the Centers for Disease Control and Prevention (CDC) and the state of New Jersey. The form outlines the procedures and practices that New Jersey will follow in conducting the 2018 BRFSS data collection, including using the BRFSS telephone sample provided by the CDC, monitoring interviewers, using the standard questionnaire, and completing required call attempts and data submissions. New Jersey indicates it will not be able to collect data for 12 full months in 2018 due to contract negotiation issues, and it will not be conducting interview verification or sending advance letters.

The related state and local contract opportunity is for the New Jersey Behavioral Risk Factor Survey (T0946), which covers Mercer County and Trenton City. The contract details are not provided in the self-certification form, but the form indicates that New Jersey will be working with a contractor to complete the BRFSS data collection.
