SECTION C – DESCRIPTION/SPECIFICATION/WORK STATEMENT

Performance Work Statement (PWS)

Title: Professional, Technical and Scientific Support Services for the Arthritis and Epilepsy

C.1 Background and Need:

The Centers for Disease Control and Prevention (CDC) is the primary Federal agency for protecting health and promoting quality of life through the prevention and control of disease, injury, and disability. CDC is committed to programs that reduce the health and economic consequences of the leading causes of death and disability, thereby ensuring a long, productive, healthy life for all people.

C.2 Project Objective:

The Arthritis, Epilepsy, and Well Being (AEWB) Branch in CDC’s Division of Population Health works to improve quality for all persons by fostering public health science and translating science into population-based interventions. Specifically, the AEWB branch provides national leadership to address conditions whose primary consequence is reduced quality of life, including arthritis and epilepsy. The program works with state health departments, national organizations, and universities to deliver programs, conduct prevention research, surveillance, and analysis. The branch also leads national efforts to provide quality of life surveillance as a key component of national, state, and community assessment. These activities focus public health chronic disease attention on conditions that have widespread or serious implications for people’s ability to ensure that their added years of life are quality years of life.

C.3 Scope of Work:

The Contractor will provide scientific, technical, and professional assistance such as: scientific and professional support to identify and establish research objectives, recommend methodology for execution of the research agenda; conduct epidemiologic research studies utilizing existing data from NHIS; conduct data analyses;

prepare manuscripts; provide statistical support.

Performance will be in accordance with this Performance Work Statement and the Quality Assurance Surveillance

Plan. Work will be performed by trained and qualified contractor employees who can successfully perform the requirements as established. It is the responsibility of the Contractor to provide all necessary training to maintain competency at no additional expense to the Government.

C.4 Technical Requirements:

1. Core Requirements

The Contractor will perform the tasks set forth below in support of the Arthritis and Epilepsy

Programs with one full time employee:

Core Labor Task 1 - Statistical/Programming Support Services

a. Provide epilepsy statistical support activities (e.g., uses epilepsy data from complex surveys such as

NHIS, BRFSS, HealthStyles, NVDRS, and School Health Profiles, to conduct data analysis using SAS, SPSS, SUDAAN or other statistical software to generate data on epilepsy burden for publications, reports, communication material or other Epilepsy Program products).

b. Supply printed reports in SAS, SPSS, SUDAAN , or MS Word relevant to the statistical support activities for review by Epilepsy Program staff.

c. Apply knowledge of statistical methods to refine statistical models, and determine statistical requirements related to different research questions developed by Epilepsy Program staff (questions will vary depending on dataset).

d. Conduct statistical analyses to generate and draft Epilepsy Program “By the Numbers” (bimonthly) feature in the journal, Epilepsy and Behavior. This feature is modelled on the MMWR QuickStats feature

(http://www.cdc.gov/mmwr/volumes/65/wr/mm6504a8.htm?s_cid=mm6504a8_w) and involves a simple analysis including one chart and an abstract. Epilepsy Program feature topics include: (Associations between active epilepsy and medication adherence; employment status; work-loss days; sick/bed-days; access to specialty care;

immunization status; veteran status; suicide, and other topics determined by CDC).

e. Conduct statistical analyses of 2010, 2013, and 2015 National Health Interview Survey data to generate updated estimates of epilepsy burden (e.g. overall prevalence, and by subgroups; comorbidity; gaps in care).

Apply these estimates to the latest U.S. Census estimates to generate state-specific estimates of epilepsy burden.

f. Conduct statistical analyses to generate methods and results section of a manuscript describing history of mental illness among adults with epilepsy who have completed suicide.

g. Identify and recommend mixed-methods approaches for program evaluation. The contractor will conduct a literature review and environmental scan of mixed-methods approaches used to evaluate public awareness programs in diabetes, smoking, HIV-AIDS, and asthma. Based on this review, the contractor will recommend mixed-methods approaches to guide evaluation of Program grantee projects.

h. Conduct statistical analysis of latest available School Health Profiles survey data to generate tables describing schools’ policies on supporting students with epilepsy.

i. Prepare and present analyses at a national epilepsy meeting.

2. Optional Tasks

The following optional tasks shall be performed on a part time basis and may be exercised and funded at the discretion of the Government. The period of performance for optional tasks will begin on January 9, 2017, in the base year and continue through the end of the base year.

Optional Task 1 – Statistical Programming Support

a) Provide SAS programming services for: gamma regression models and other complex econometric and statistical techniques as needed for up to 3 analyses.

b) Provide statistical consultation on choice of statistical replication methods (balanced repeated replication, bootstrapping) for analysis of complex survey data including Medical Expenditure

Panel Survey data for up to 3 analyses; this includes SAS programming services.

c) Review up to 6 manuscripts to assess the adequacy of statistical methods used and to confirm that the results are appropriate to the methods.

Optional Task 2 – Statistical Support for Research

a) Refine complex and sophisticated statistical methods and models being used, and determine statistical requirements related to up to 3 selected research questions.

b) Conduct subsequent statistical analyses for each of the projects identified above and produce tables and figures of results.

c) Program and generate statistical analysis and reports in SAS/SUDAAN for up to additional 3 projects.

d) Update existing SAS/SUDAAN programs and include detailed documentation for the 2015

Behavioral Risk Factor Surveillance Survey (BRFSS) to produce arthritis standard tables for states and territories, and documenting those updates in the program so existing Program staff are capable to running these programs.

Note: Contractor shall perform the same tasks for base period and all option periods, if exercised.

Minimum Education/Experience or Equivalent – Applicable to the Core Tasks and all Optional Tasks

Statistician with coursework/training beyond the master’s degree level with proficiency and experience in:

- statistical programing, using the latest SAS programming version(s) and SUDAAN programs, and manipulation of large data sets;

- statistical methods including proportional weighting, generalized linear models, linear and logistic regression (including model specification, estimation of model parameters and standard errors, evaluation, interpretation, and inference);

- variance estimation (including replication methods);

- hypothesis testing;

- analysing longitudinal and survival data (including interval-censored data) based on complex survey designs

- general multilevel models, assumptions, familiarity with analytic approach, ability to identify and proficiency with use of appropriate software

2. Place(s) of Performance

Core Labor Task 1 – Services will be performed on site at the Centers for Disease Control and

Prevention, National Center for Chronic Disease Prevention and Health Promotion, Division of

Population Health, Chamblee Campus, Atlanta, Georgia 30341, unless otherwise indicated.

Optional Tasks 1 and 2 – Services will be performed at Contractor’s facilities

3. Government-Furnished Equipment (GFE), Laptop and/or Facilities Provided

The Government will provide on-site contractor personnel with an office environment typically provided to Government personnel that includes workstations, facsimile, copiers, telephones and computers with access to the Internet and local area network (LAN). The government will also provide an identification badge for access to the building(s) and CDC property upon successful completion of all safety and security training and security clearance requirements.

4. Custodial Account

Contractors are required to obtain a custodial account number from the CDC Property Custodian for

Government-Furnished Equipment (GFE). Only a CDC Contracting Officer may authorize the release of equipment to contractors for removal from CDC facilities. The property must be included in the contract as part of the itemized list of property being provided.

5. Travel

If travel is required for the core tasks the contractor’s attendance may be required to include a presentation at one national epilepsy meeting.

C.6. Special Requirements

508 Clause

Section 508 of the Rehabilitation Act (29 USC 794d)

Section 508 of the Rehabilitation Act of 1973 (29 U.S.C. 794d), as amended by the Workforce

Investment Act of 1998, and the Architectural and Transportation Barriers Compliance Board Electronic and Information (EIT) Accessibility Provisions (36 CFR part 1194), require that, unless an exception applies, all EIT products and services developed, acquired, maintained, or used by any Federal department or agency permit:

(1) Federal employees with disabilities to have access to and use information and data that is comparable to the access and use of information and data by Federal employees who are not individuals with disabilities; and

(2) Members of the public with disabilities seeking information or services from a Federal agency to have access to and use of information and data that is comparable to the access and use of information and data by members of the public who are not individuals with disabilities.

Accordingly, any vendor submitting a proposal/quotation/bid in response to this solicitation must demonstrate compliance with the established EIT accessibility provisions. The Section 508 standards applicable to this solicitation are identified in the Statement of

Work/Specification/Performance Work Statement. In order to facilitate the Government’s evaluation to determine whether EIT products and services proposed meet applicable Section 508 accessibility standards, offerors must prepare an HHS Section 508 Product Assessment Template, in accordance with its completion instructions, and provide a binding statement of conformance. The purpose of the template is to assist HHS acquisition and program officials in determining that EIT products and services proposed support applicable Section 508 accessibility standards. The template allows vendors or developers to self-evaluate their products or services and document in detail how they do or do not conform to a specific

Section 508 standard. Instructions for preparing the HHS Section 508 Product Assessment Template may be found at http://508.hhs.gov.

Respondents to this solicitation must also provide any additional detailed information necessary for determining applicable Section 508 standards conformance, as well as for documenting EIT products and/or services that are incidental to the project, which would constitute an exception to Section 508 requirements. If a vendor claims its products and/or services, including EIT deliverables such as electronic documents and reports, meet applicable Section 508 standards in its completed HHS Section

508 Product Assessment Template, and it is later determined by the Government – i.e., after award of a contract/order, that products and/or services delivered do not conform to the described accessibility in the

Product Assessment Template, remediation of the products and/or services to the level of conformance specified in the vendor’s Product Assessment Template will be the responsibility of the Contractor at its expense.

IT Security

IT Contractor performance and resulting deliverables must adhere to all federal, HHS, and/or CDC IT security policies and procedures.

Development or implementation of an electronic information system or any electronic data collection effort conducted in the performance of this contract will be required to complete Certification and

Accreditation (C&A) prior to operation resulting in an Authority of Operate (ATO) from CDC. The contractor shall be required to complete all security documentation and materials necessary to obtain an

ATO. The contractor shall comply with all applicable HHS, CDC, FISMA, HIPPA, NIST, and other federal policies and regulations in the performance of the security requirements.

All information systems developed, implemented, or maintained in support of this contract must adhere to the security controls outlined in the National Institute of Standards and Technology (NIST) Special

Publication 800-53, Recommended Security Controls for Federal Information Systems and Organizations

The contractor must follow CDC development standards and use the CDC web template package. The

CDC Secure Web Application Coding Guidelines and CDC Template Package

(http://intranet.cdc.gov/ociso/pandp/documents/CDC_Secure_Web_App_Coding_Guidelines.pdf) can be provided by the CDC Project Officer or NCCDPHP ISSO.

During any application development or enhancement, the contractor shall work closely with the

NCCDPHP Office of Informatics and Information Resources (OIIRM) to ensure all CDC-related technical and security standards, processes and procedures are followed. This working relationship shall continue through full production.

All web content must reside on a CDC.gov domain and must be hosted on CDC or CDC-authorized information systems. The contractor may not establish an .org or .com site to host CDC funded content or web applications. All CDC websites, campaign sites, and applications must use standard the standard

CDC web template to ensure a consistent look and feel for CDC websites. The template will be provided as needed.

CDC information systems are not approved for cloud deployment at this time (May 2015). However, if cloud deployment is approved in the future, the provider must be FedRAMP certified. The contractor shall comply with FedRAMP requirements as mandated by Federal laws and policies, including making available any documentation, physical access, and logical access needed to support this requirement. The

Level of Effort for CDC’s authorization is based on the System’s NIST Federal Information Processing

Standard (FIPS) Publication 199 categorization.

All publicly accessible Federal websites and web services must provide service over a secure connection.

The strongest privacy protection currently available for public web connections is Hypertext Transfer

Protocol Secure (HTTPS).

Capital Planning and Investment Control/Project Management

The Federal Government mandates the prudent management of IT investments. Capital Planning and

Investment Control (CPIC) is a continuous and integrated process for managing the risks and returns of information technology (IT) investments. The CPIC process fully integrates with the CDC’s overall budget, finance, acquisition, strategic planning, enterprise architecture, security, and other relevant processes. CPIC also aligns with DHHS Enterprise Performance Life Cycle (EPLC) framework and is used for all IT related decisions.

The contractor will follow the EPLC framework which will provide a standard structure for planning, managing, and overseeing IT projects over their entire life cycle. The framework consists of ten life cycle phases. Within each phase, activities, responsibilities, reviews and deliverables are defined. Templates for the deliverables are available to the contractor after award. Exit criteria are established for each phase and

Stage Gate reviews are conducted through CDC’s IT Governance process to ensure that the project’s management quality, soundness, and technical feasibility remain adequate and the project is ready to move forward to the next phase. All IT projects in support of this task must adhere to the EPLC requirements and pass each State Gate as appropriate. More information about EPLC can be found at http://www.hhs.gov/ocio/eplc/.

Mobile Application Development

All mobile applications must comply with CDC-related technical and security standards, processes and procedures as stated in the IT Security section of the RFTOP. In addition, the following considerations should be addressed in the proposal:

The target platform(s) for the mobile application (e.g. iPhone, iPad, Android, Blackberry)

Approach to 508 compatibility for the platform of choice

If the mobile application will be made available in a related platform’s online store (e.g. iTunes, Android Market), the process for making it available in a desired store will be managed by and through CDC, resulting in a CDC branded mobile application. The contractor will also be responsible for ensuring the mobile application complies with all standards and specifications for the desired store.

Records Retention

All data created during the course of this contract must be retained, stored and disposed of in accordance with the appropriate CDC Records Control Schedule.

Data Collection

The contractor should collect or create only that information or data necessary for the proper performance of CDC functions and which has practical utility. Limit the collection or creation of information which identifies individuals to that which is legally required, authorized, and necessary for the proper performance of CDC functions. Data collections should only include the minimum information reasonably necessary to conduct public health activities. Unless there is a valid public health purpose, personally identifiable data (i.e., name, date of birth, phone number) should not be collected or maintained. Prior to implementing data collections, minimum data elements should be specified and consideration should be given to whether data collections of identifiable data will be necessary to achieve the intended public health goals.

To ensure the issues of confidentiality, proprietary use, and informed consent are correctly addressed, a data management plan must be initiated during the project planning phase as to how the contractor will collect, analyze, distribute, and make available the data collected.

Schedule of Milestone for Performance-Based Payments

In accordance with FAR 52.232-28 (Invitation to Propose Performance-Based Payments) and FAR 52.232-32, PERFORMANCE BASED PAYMENTS (April 2012), upon successful completion of an event, the contractor may request performance based payments. The determination of eligibility for receipt of payment will be made by the Contracting Officer upon written certification from the Contracting Officer’s Representative (COR) that the performance milestone has been met. It is anticipated that each milestone payment will approximate the estimated timeline listed below but variation is anticipated and eligibility for payment could occur sooner or later depending upon the time of completion of the designated milestone. Milestone payments are subject to the terms of FAR 52.232-32 incorporated in Section I of the basic contract

The COR receiving and acceptance of performance based payments is based on verification that work on the contract was satisfactorily performed.

CDC proposes the Milestone Payment Schedule for this contract as outlined below. As part of the negotiated process, however, the contractor may provide a different milestone schedule. If you decide to agree with the milestone schedule as outlined below, you should provide your proposed payment percentage allocation for each scheduled payment below based upon your total proposed price. The final milestone payment schedule will be negotiated between the parties and included in the contract award. However, if the parties cannot agree the Contracting Officer shall make the final decision.
