About this file

This document is a sworn statement form required pursuant to Kentucky Revised Statute KRS45A.395 for bidders or offerors submitting proposals to the City of Owensboro, Kentucky. The form serves as a prerequisite for determining whether a bidder or offeror qualifies as a responsible bidder under Kentucky procurement law. The document does not specify particular products, services, quantities, response dates, or contract terms, as it appears to be a standard compliance form that would accompany various solicitations from the City of Owensboro's Purchasing Department.

The statement requires bidders to affirm under penalty of perjury that neither they nor anyone acting on their behalf has knowingly violated Kentucky's campaign finance laws, and that awarding them a contract would not violate these laws. The form includes signature fields for company representatives to complete, including company name, authorized signatory, signature, and title, along with a date field for 2025. This compliance requirement appears to be a standard component of the City of Owensboro's procurement process to ensure ethical contracting practices in accordance with Kentucky state law.
