About this file
This is a sworn statement form required by Kentucky Revised Statute (KRS) 45A.395 for bidders and offerors seeking state and local contracts in Kentucky. The statement serves as a prerequisite to establishing bidder responsibility and must be completed and submitted by any party bidding on Kentucky contracts.
The sworn statement requires the undersigned, acting in an official capacity on behalf of the bidder or offeror, to affirm under penalty of perjury that neither they nor any person acting on the bidder's behalf has knowingly violated Kentucky's campaign finance laws. The statement further certifies that awarding the contract to the bidder or offeror will not violate any provision of the Commonwealth's campaign finance laws. "Knowingly" is defined as awareness or the reasonable expectation of awareness that conduct is of a particular nature or that a circumstance exists. The form includes blank fields for the signatory to enter the date, company name, signature, title, office or title, and bidder or offeror information. This certification applies to all state and local contract opportunities in Kentucky and represents a mandatory compliance requirement for bid responsiveness.
