About this file

This document is a Statement Required Pursuant to KRS45A.395 from the City of Owensboro, Kentucky, located at 101 E. 4th Street. The form is a sworn statement required by Kentucky state law that bidders or offerors must complete as a prerequisite to being considered a responsible bidder for municipal contracts. The statement requires the individual signing to affirm, under penalty of perjury, that neither they nor anyone acting on their behalf has knowingly violated any campaign finance laws of the Commonwealth of Kentucky.

The document provides a template for bidders to complete, including spaces for the company name, signatory, signature, title, and date. The key legal requirement is that the bidder certifies they are aware of and have not violated campaign finance laws, with "knowingly" defined as being aware or should have been aware of potential violations. The form is part of the City of Owensboro's Purchasing Department's procurement process, as indicated by the department's contact information and logo at the top of the document.
