About this file
This is a Required Forms document package for the City of Fort Worth's RFP 26-0225 for Elevator and Escalator Maintenance and Repair services. The solicitation seeks proposals for non-exclusive agreements to provide elevator and escalator maintenance and repair services on an as-needed basis for the Property Management Department and Public Events Department. The RFP requires proposers to complete multiple forms including company information, a detailed questionnaire addressing service capabilities, the IRS Form W-9 for tax identification purposes, and a Texas Conflict of Interest Questionnaire. Key service requirements outlined in the questionnaire include 24/7 emergency response capability, documentation of maintenance activities through a web-based service management portal or equivalent electronic system, and the ability to provide both elevator and escalator services that align with Sections 2.0 through 21.0 of the Scope of Services. Proposers must identify the number of maintenance technicians assigned to the contract, office and service center locations supporting the contract, and clarify whether they provide elevator-only, escalator-only, or both services.
The questionnaire addresses prompt payment discount terms and requires proposers to disclose any litigation, criminal investigations, or proprietary information claims related to their submission. Proposers must also confirm their willingness to extend contract terms, conditions, specifications, and pricing to other governmental entities participating in cooperative purchasing arrangements. The document does not specify pricing structures, contract values, estimated quantities, or renewal options within the forms package itself. No set-asides for disadvantaged or small businesses are indicated in the forms document. The submission package requires completion and filing of the Conflict of Interest Questionnaire with the City Secretary no later than seven business days after beginning contract discussions, in compliance with Texas Local Government Code Chapter 176, with failure to comply constituting a Class C misdemeanor.
