About this file
This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form required by the Los Alamos County Procurement Division in New Mexico for the Food Waste Composting Services solicitation. The form serves as a certification document that bidders and offerors must complete and submit as part of their proposal package. Offerors are required to certify their eligibility to contract with the County by confirming they are not debarred, suspended, proposed for debarment, or otherwise ineligible from covered transactions at any federal, state, or local level. The form must be signed by an authorized representative of the offeror company and submitted with their bid or proposal response.
The certification requires offerors to affirm that neither they nor any of their principals have, within the preceding three years, been convicted of fraud, criminal offenses related to public transactions, antitrust violations, embezzlement, theft, forgery, bribery, or falsification of records. Offerors must also confirm they have not violated contract provisions, failed to perform satisfactorily on prior contracts, violated ethical standards under the County Procurement Code, or had public contracts terminated for cause or default. Additionally, offerors must certify they are not current or former County employees and are not immediate family members of County employees or public officials as defined by the form. Any inability to certify these statements requires attached explanation, and falsification of any statement constitutes grounds for proposal rejection or contract award rescission.
