About this file
This document is a Louisville Metro Codified Ordinance (LMCO) Policy Certification form relating to hiring practices for vendors doing business with Louisville Metro Government. The ordinance, specifically Section 112.30, prohibits vendors from using employment applications that inquire about an applicant's prior criminal history on initial applications, with several specific exceptions. These exceptions include situations involving handling significant cash amounts, access to confidential information, unsupervised contact with children or vulnerable populations, and positions that may be legally restricted based on criminal history.
The policy mandates that the criminal history guidelines will be part of contract evaluation criteria, and the City retains the authority to deny, rescind, revoke, or not renew contracts with vendors who fail to comply with these standards. Vendors must affirm they either follow the policy or qualify for one of the listed exceptions. The document provides a certification form where vendors can indicate applicable exceptions and sign to acknowledge their understanding of the requirements, with spaces for an authorized official's signature, name, title, and date of certification.
