About this file

This is a Request for Proposals (RFP) document and cost proposal template issued by City Colleges of Chicago for RFP #NC2608 covering life, accidental death and dismemberment (AD&D), critical illness, and disability insurance. The RFP seeks proposals for employer-paid group term life insurance for eligible full-time active employees and retirees, along with voluntary supplemental coverage options. The proposal encompasses basic life and AD&D coverage, voluntary supplemental life and AD&D by age brackets, voluntary AD&D coverage, short-term disability, long-term disability, and critical illness insurance. As of December 2025, the total volume of coverage requested includes $277.97 million in basic life and AD&D, $93.3 million in employee supplemental life coverage, and additional voluntary coverage across multiple product lines. Proposals must be submitted no later than Monday, March 30, 2026 by noon Central Standard Time.

The RFP requires proposers to complete detailed interrogatories addressing general qualifications, administrative and operational capabilities, customer service delivery, claims processing procedures, financial stability, and underwriting systems. Vendors must demonstrate Illinois licensure, provide three years of financial stability ratings, and disclose enrollment data for the metropolitan Chicago area, Illinois, and nationwide markets. The proposal structure mandates that pricing be submitted on the provided Excel template without format alterations, with separate cost proposal tabs for each insurance product line organized by employee classification, age bracket, and coverage type. Proposers must document their ability to transition current enrollees, process evidence of insurability online when possible, provide monthly billing and claims analysis, maintain service levels including toll-free customer service with specified hours, and submit claim status reports electronically.
