About this file
This document is a Conflict of Interest Questionnaire (CIQ) from the City of Fort Worth Purchasing Division for Invitation to Bid (ITB) Reference #25-0231, which pertains to Ready-Mix Concrete and Flowable Fill. The document outlines requirements under Chapter 176 of the Local Government Code for vendors doing business with local governmental entities. Vendors must disclose any business relationships or potential conflicts of interest when contracting or seeking to contract with the City of Fort Worth, and must file the CIQ form no later than seven days after beginning contract discussions or submitting a bid response.
The CIQ form requires vendors to provide detailed information about their business relationships, including any connections with local government officers or their family members that might result in taxable income. Specific disclosure requirements include reporting any employment or business relationships, gifts, or financial transactions exceeding certain thresholds. The form must be completed accurately, with vendors required to check specific boxes indicating the nature of their potential conflicts of interest, such as receiving income from the local government entity or having business relationships with government officers. The Texas Ethics Commission provides this standardized form to ensure transparency in local government contracting processes.
