About this file

This document is a Conflict of Interest Questionnaire (CIQ) issued by the City of Fort Worth Purchasing Division for Invitation to Bid (ITB) Reference #25-0209, titled "Moving & Disposal of Gaming Machines". The CIQ is a standardized form required pursuant to Chapter 176 of the Local Government Code, which mandates that any person or agent seeking to contract with a local governmental entity must disclose potential conflicts of interest. The form must be filed with the City of Fort Worth Secretary no later than seven days after the person begins contract discussions, negotiations, or submits a bid or proposal.

The questionnaire requires vendors to provide detailed information about their business relationships with local government entities, including any employment or financial connections with government officers or their family members. Specifically, vendors must disclose if they or their family members receive taxable income exceeding $2,500 from the local government entity in a 12-month period, or if they have been given gifts valued at $100 or more. The form is designed to ensure transparency and prevent potential conflicts of interest in governmental contracting processes. Vendors are instructed to complete all relevant sections, with a signature required in section 7 even if no conflicts are identified.
