About this file
This is a Foreign Entity Ownership Affidavit required for bidding on contracts with the State of Florida, specifically for the RFP for Disaster Debris Management. The affidavit must be submitted with every bid or offer on contracts that provide entity access to personal identifying information. The document requires the entity to attest under penalty of perjury to multiple conditions regarding foreign ownership and control, including that the entity is not owned by or organized under the laws of a foreign country of concern as defined in Florida Statutes Sections 287.138 and 692.201, does not have a controlling interest held by a foreign government, is not a foreign principal, and maintains compliance with all applicable requirements outlined in Sections 692.202, 692.203, and 692.204 of the Florida Statutes. For contracts involving the purchase of real property, the affidavit additionally requires attestation that the entity is not prohibited from purchasing the subject property under applicable law.
The affidavit must be signed by an authorized representative of the bidding entity, with the signature, printed name, title, entity name, and date required on the document. This attestation is a mandatory compliance document for participation in Florida state and local procurement processes and serves as a screening mechanism to ensure that all contractors meet foreign ownership and control restrictions established under Florida law. Failure to provide this affidavit or providing false attestations may result in bid disqualification or contract termination.
