About this file

This is a Felony Conviction Notification form required by the State of Texas for vendors contracting with Manor Independent School District. The notification requirement is mandated under Texas Legislative Senate Bill No. 1, Section 44.034, which requires persons and business entities entering into contracts with school districts to provide advance notice if they or their owners/operators have been convicted of a felony, including a general description of the conduct resulting in the conviction. The form applies to the District's Request for Proposals for commercial freezer and cooler preventative maintenance and repair services for school kitchen facilities across the District. Responses must be submitted to Manor Independent School District's Purchasing Coordinator, Chrissie Bryant, via the Euna portal, with inquiries directed to Purchasing@manorisd.net.

The notification form provides three certification options for vendors: confirmation that the firm is a publicly held corporation and therefore exempt from the reporting requirement; certification that the firm is not owned or operated by anyone convicted of a felony; or disclosure of individual owners or operators with felony convictions and details of those convictions. The District retains the right to terminate any contract if it determines that a vendor failed to provide required notice or misrepresented information regarding convictions, though the District must compensate the vendor for services performed prior to termination. All authorized company officials must sign the certification under penalty of providing false information.
