About this file
This is a Drug-Free Workplace Affidavit form that must be completed by firms responding to state and local contract opportunities. The document requires the undersigned to certify that their firm has implemented comprehensive drug-free workplace policies and procedures. The affidavit mandates that responding firms have taken six specific steps: publishing a statement prohibiting controlled substance activities in the workplace, informing employees about drug abuse dangers and available assistance programs, distributing the drug-free workplace statement to all employees engaged in the proposed work, notifying employees of their obligation to report drug-related convictions within five days, implementing sanctions or requiring rehabilitation participation for convicted employees, and maintaining ongoing good faith efforts to preserve a drug-free work environment.
The affidavit serves as a mandatory compliance document for contractors bidding on government contracts, ensuring that all participating firms maintain appropriate workplace safety standards and drug prevention policies. The form requires certification that employees working specifically on the commodities or contractual services under bid will abide by these drug-free workplace requirements as a condition of their employment. This document represents a standard requirement in government contracting to promote workplace safety and compliance with federal and state controlled substance laws.
