About this file

This Statement of Work details Fermilab's requirements for safety shoe footwear procurement for employees at their Batavia, IL laboratory. The vendor must provide ASTM F2413 compliant safety toe footwear through a "shoe mobile" stationed on-site every other Tuesday from 8:00 a.m. to 4:00 p.m., offering at least eight men's and six women's styles from brands like Avenger, Keen, Wolverine, and Puma. Fermilab will cover up to $200 per pair of footwear, with employees responsible for any costs exceeding that allowance.

The document specifies detailed requirements including size ranges (men's 7-14, women's 5-12), width options, and estimated annual usage for each style. Additional provisions include on-site fit improvements, exchanges, and returns at no extra cost. The contract includes a base period and one option year, with separate pricing schedules for standard safety footwear (Schedule C) and occupational/medical footwear options (Schedule D), such as insulated, waterproof, electrically rated, and orthotic-compatible styles to accommodate specialized workplace needs.

SAFETY SHOE FOOTWEAR

STATEMENT OF WORK

The vendor will provide ASTM F2413 compliant safety toe footwear to Fermilab employees at the Batavia, IL laboratory location as described below and listed in Schedules C: Safety Shoe Price Schedule. Medically prescribed corrections and occupationally required options for ASTM F2413 compliant footwear are listed in Schedule D: Occupational/Medical Footwear Price Schedule and must be available as an option when authorized.

Footwear will be dispensed on the Fermilab site by the vendor. Dispensing will be conducted on every other Tuesdays (from 8:00 a.m. to 4:00 p.m.). The vendor will provide an adequate number of personnel to provide continuous service coverage for the entire eight-hour period.

The dispensing facility will be a “shoe mobile” provided by the vendor. The vendor must provide personnel who can perform fit improvements while on-site through the use of a roller, stretchers, pads, or insoles at no additional cost to employees. Exchanges, returns, and credits shall also be the responsibility of these personnel.

The vendor will provide footwear to employees who present a properly completed Fermilab Safety Toe Footwear Request form. The form shall be filled in correctly with all information legible, including the employee’s name and budget code, and shall have the necessary approval signatures indicating the type of safety footwear to be provided to the employee. Fermilab will pay up to $200.00 (allowance) per pair of footwear, as well as additional costs for any medically prescribed corrections or occupationally required options. Employees will directly reimburse the vendor for any other costs.

The vendor will provide at least eight styles of men's footwear and six styles of women's footwear, with a variety of brand name safety footwear. The footwear will be stocked by the vendor in the dispensing facility in a range of sizes which may be expected to accommodate 90% of the population. Employees must be able to choose from the available styles. Employees may also select other styles from the vendor that are ASTM F2413 rated and within the $200.00 allocation (allowance), or they may pay the difference directly to the vendor for options costing more than the allocated amount.

The styles offered on the truck must be similar and the quality equivalent or superior to that of the footwear listed below. For medically prescribed corrections and occupationally required options, see Schedule D.
