1. Minimum clearing limits shall be as indicated on plans.

2. All dimensions are in U.S. feet.

3. The contractor is responsible for all project safety and safety compliance.

4. Limited operating periods are listed in the FSSS Section 107.

5. Provide copies of weight tickets for material delivered to the site from commercial sources. Aggregate shall be obtained from an approved source, certified weed free, and certified to contain no more than 0.25% asbestos to be in compliance with California Health and Safety Code Sections

93105 and 93106.

6. Borrow source: If needed, will be designated during construction by the Forest Service. Any material to be available at a borrow site will be unclassified borrow. Material may need to be sorted or mixed to meet gradation requirements.

7. Potential water sources: Bear Creek at 3N01 South crossing or drainage channel 0.1 miles prior to Reed Creek at 1N04 crossing. Meet in-stream drafting minimum flow requirements. When drafting from the river, use screening devices for water drafting pumps. Use pumps with low entry velocity to minimize removal of aquatic species, including juvenile fish, amphibian egg masses and tadpoles, from aquatic habitats.

8. Any merchantable timber needed to be removed to facilitate the road repair shall be marked by the Forest Service prior to felling. Perform felling, bucking, and decking of merchantable timber designated for removal. Operation shall use standard logging practices to minimize breakage of utilizable logs and protect residual trees from unnecessary damage. Merchantable timber shall be bucked into standard log lengths with proper trim allowance prior to decking.

9. Hazard trees: the contractor is responsible for mitigating any work hazards at the project site. Hazard trees can be directionally felled away from the road and left in place.

10. When State of California specifications are referred to, download is available at:

http://ppmoe.dot.ca.gov/hq/esc/oe/construction_contract_standards/std_specs/2018_stdspecs/2018_stdspecs.pdf

4. The contractor shall submit a traffic control plan in accordance with FP-14 Section 156 to the C.O. for approval prior to the start of work.

5. Required work not specified in the schedule of items shall be considered incidental to other pay items.

6. Before any excavation activities begin, the contractor shall contact a service to locate any buried utilities. USA 1-800-227-2600, the national 811 number, or the website http://www.call811.com.

7. Equipment, for this project, if driven on state or county roads shall be currently licensed.

8. All work shall be in compliance with the best management practices described in the

Region 5 Forest Service Soil and Water Conservation Handbook, dated December 2011, and the National Best Management Practices for Water Quality Management on National

Forest System Lands, dated April 2012.

Erosion Control Notes:

1. The contractor shall obtain coverage under the General Permit for Discharges of Storm Water Associated with Construction Activity under the Construction General Permit Order 2009-0009-DWQ as administered by the California State Water Resources Control Board. To obtain coverage under the General Permit, the contractor shall electronically file the Permit Registration Documents (PRDs), which includes a Notice of Inent (NOI), Storm Water Pollution Prevention Plan (SWPPP), and other compliance related documents with the California State Water Board.

2. Provide the Forest Serivce with a copy of the SWPPP and all compliance related documents.

3. The areas of soil and vegetation disturbance shall be limited to that required for construction purposes.

except where required for access, there shall be no disturbance in areas to be left in a natural state.

4. Stockpiles shall be located in already disturbed and flat areas, and shall be surrounded with fiber rolls. If a stockpile is inactive for 14 days, it shall be stabilized with 6 mil. plastic or approved equal.

5. Any new disturbances made outside road corridors (such as staging areas) shall be restored. At a minimum, compacted areas shall be de-compacted to a 12" depth, re-contoured to match natural surrounding grade, and mulched with 2-4 inches of wood chips.

6. If tracking is evident due to construction, contractor shall broom excess sediment to an area it won't be carried away by storm flows. if this proves inadequate, contractor may have to street sweep.

7. Spill kits with absorbent pads shall be on site at all times in preparation for equipment leaks. All vehicle maintenance shall take place at least 100 feet from surface water and on a level area surrounded by a contour berm and trench.

1. Roadway rehabilitation consists of the following work:

1.1. Remove the entire depth of AC surface course. Milled AC material may be processed to a maximum particle size of 2" and stockpiled at MP 18.92 or removed off forest.

1.2. Re-shape and re-compact full depth of existing base course. Place new aggregate base where needed to level the base and structurally support the new surface course. Compact according to FSSS 301.05(b).

1.3. Construct the new AC surface course as shown in the typical section on sheet

C5.

1.4. Stripe new centerline and fog lines.

2. Begin/End rehabilitation callouts mark the full depth repair points. Taper transition to existing asphalt outside of these limits according to detail 2, sheet C5.

3. New AC surface course shall match existing alignment, profile, transitions, cross slopes and superelevation of the existing pavement.

4. Patch AC according to detail 1, sheet D4.

Work Breakdown:

1. Site #2a - Mandatory work bid items: Between STA 987+91 and STA 1035+63

2. Site #2b - Optional work bid items: Between STA 1040+67 and STA 1061+28

Asphalt Pavement Partial Depth Patch, Type 2

Work Notes:

1. Extend the repair area 12 inches beyond the distress area. Sawcut repair limits to provide vertical sides in reasonably sound pavement. Remove all asphalt pavement within the repair limits down to the existing base course. Dispose of asphalt off project.

2. Clean the patch area by sweeping or other acceptable methods. Remove water and debris from pothole.

3. Compact existing base course or subgrade with vibratory plate compactor or similar.

4. Apply a tack coat to asphalt concrete surfaces within the patch area.

5. Place the asphalt concrete. Placed depth shall match the adjacent existing asphalt section.

6. Compact the asphalt concrete pavement patch to match the grade of the adjacent surface.

Crown top of new asphalt above existing road surface to prevent water pooling.

7. Asphalt sawcut, removal, tack coat and new asphalt concrete for patching are incidental to the pavement patch line item.

2. The exterior screen mesh may be either rigid or flexible and made from durable material.

3. Stop use of the screen if any holes larger than

" diameter develop in the exterior screen. Use can be resumed after repair.

Work Notes:

1. Signs are shown for one direction of travel only. Place devices similar to those depicted for the opposite direction of travel.

2. Final location and spacing of signs and devices may be changed to fit field conditions as approved by the CO.

3. For pilot car operation, mount the PILOT CAR FOLLOW ME (G20-4) sign at a conspicuous location on the rear of vehicle. Prominently display the name of the contractor on the pilot car.

4. If closure is completely within the project limits, eliminate the "ROAD WORK AHEAD" (W20-1) and "END ROAD

WORK" (G20-2) signs.

5. For night time flagging operation, provide floodlighting at flagger stations.

6. For project specific minimum width, refer to the Special Contract Requirements, Section 156.

7. Do not allow equipment, materials, or vehicles to be parked or stored in the buffer space.
