Tires are free from cracks, cuts, gouges, bumps or any tread or sidewall separtation. No tire belts or plys exposed through the tread or sidewall of tire. Thread depth must be equal to or greater than 8/32".

All axle "Zerk" grease fitting covers present.

All axles have no visual structural damage, being bent or rust.

Recommendation:

6. Temperature and Climate conditions

Brake axle wiring harness does not have exposed wires and can be reconnected to another towing vehicle.

Under the axle leaf springs have no visual structural damage or visual sagging.

Drain line drop location is not less than two feet (2’) but not more than three feet (3’) in front the first axle from the front of the MHU.

3/4" main water inlet 1) located within 12" of the curb side of the MHU, 2)note more than 6" below the bottom board, 3) has cap and 4) master shut off valve installed.

Drain line drop shall 1) protrude at least six inches (6”), but not more than eight inches (8”), from the bottom board, 2) shall have a threaded end, and 3) be capped with a removable plastic cap and chain.

Fail Notes

Bottom board tire blowout protection in place above axles, both sides without visual damage.

Chassis component welds are not cracked. Weld slag is removed.

Suspension system components installed with locking nuts or lock washers.

All exposed surfaces of entire frame are painted and no rust observed.

NEMA rated electrical service entrance junction box mounted to floor joist or I-beam.

Junction box is rated NEMA 3 or better and within 10' of curbside rear of unit without blocking exit. Minimum size for junction box is 12” x 12” x 4.”

Bottom board is 1) secured, 2) without exposed tears or rips and 3) access panels have identification label and are secure to bottom board.

Bottom board repair of tears and/or rips is secure to bottom board anot hanging down. No exposed insulation.

14 Wide Furnace only - 3" dia. X 6" PVC protective sleeve pipe friction fitted into rear header.

Heat tape receptacle has weather proof cover and is within 24" of the the water inlet.

8 Wide Express - Exterior TPS Water Connection - On right side, approximately 8 feet from rear end of MHU, 1 1/2" male cam locking fitting with locking dust cap, extended thru bottom board.

8 Wide Express - Sprinkler Drain - On right side, approximately 8 feet from rear end of MHU, 1" black iron drain pipe and dust cap, extended thru bottom board to exterior perimeter of MHU.

Front and rear headers have 10 lags evenly spaced along top of I-Beam header. Lags are tight and cannot be moved with hand.

All lag clips welded to chassis I-Beam have lag screws and cannot be moved. (lag clips in axle area are on the inside face of the main I-beam)

Longitudinal brackets located on bottom of main I-beam per DAPIA approved chassis drawing. Welds are not cracked, slag removed and no rust.

All chassis components free from visual damage (i.e. flange bent on I-

Beam, cross members, outriggers, etc.), painted and without rust.

8' Wide Express - The external power supply for external TPS system shall be 1) a female IEC 60 309-2 International pin and sleeve connection, 2) located on rear of MHU 24" in from the road side of the

MHU and 3) 12" up from the bottom of the exterior siding.

1/2" in height, 2) vinyl bulb primary seal 3) a secondary vinyl weather strip.

Vinyl siding is: 1) light gray color and 2) double 4" lap configuration.

Soffit, trim, fascia, J-channel shall be white or same color as the vinyl siding.

Trim above doors shall not have weep holes or joints.

All exterior wall penetrations, such as windows, doors, etc., are caulked to prevent air and moisture penetration.

All windows, except Kitchen window, are 30" x 60" windows.

All windows have certification label stating 1) Dual pane, 2) Low E, 3)

SHGC = .30, 4) U Value = .36 or less and 5) Design Pressure = 58 or greater.

Exterior entry doors are: 1) 36” x 80”, 2) hinged toward front of MHU, and

3) do not swing into windows when doors are fully opened.

The two exterior entry doors are not be on the same side.

Exterior entry doors shall have: 1) a peephole 5' 3" +/- 1/2" from finished floor and 2) a peephole 3' 7" +/- 1/2" from finished floor. (TPS doors do not require a peephole.)

Vendor MHU serial number shall also be stenciled in white or contrasting color, 2” tall on the hitch. Serial number shall match serial number recorded on the HUD data plate.

Vendor MHU serial number stamped on front header shall be clearly legible.

Coupler is welded to hitch I-beam continous, without slag and painted with rust preventative paint.

Each hitch I-beam welded to front cross member at two locations.

Each hitch I-beam welded to frame I-beam at two locations.

All hitch welds are continuous, without slag and painted with rust preventative paint.

MHU Identification

Walls

Kitchen window - Fixed 30" x 27" window.

FEMA MHU number placed on exterior left front corner 3" from the bottom of adjacent roof eve and no more than 3" from left edge of unit.

Black letters are 3" in height, on white background and made of reflective material.

Nose plate installed at front of hitch.

12" I-Beam hitch reinforcement members located directly underneath front header and 1/2 the distance between the frame front header and the coupler.

HUD label placed on roadside rear elevation of MHU.

Drawbar is painted Green (indicating CONUS Unit).

Screw jack is top mount and has a galvanized coating.

Hitch Assembly

Left and Right side elevations shall have tie down brackets no more than

2' from each end and spaced at:

8' Wide Express - 4'-0" maximum.

14' Wide MHU's - 5' - 4" maximum.

Hitch assembly centered across front header.

All exterior entry doors shall have: 1) lever handles with a key lock and 2) key operated dead bolt. Keys shall be master keyed.

Exterior light is 1)located on the lever side of all exterior doors and 2) light fixture cover is weather tight (i.e. caulked or foam gasket).

Exterior duplex receptacle shall have: 1) weatherproof cover, 2) labeled

GFCI, 3) mounted on the lever side of the left side entry door and 4) 6" +/-

1" above the finished floor line.

Ridge Vent centered on length of MHU.

Roof

Roof penetrations capped with a galvanized metal cap.

Roof shingles are white or light colored composition shingle.

Galvanized rain diverter located above all exterior doors 3 shingle courses above metal drip edge. See Appendix A, Page A-30.

Exterior GFCI receptacle with weatherproof cover located next to exterior

HVAC component.

Metal drip edge at eave and rake end of MHU shall 1) be white, 2) not bent or sticking out from fascia, 3) have no joint above doors or windows and 4) be flush against next drip edge at joint.

1. Sprinkler Certification of Acceptance Test Form - Signed & Dated.

(FEMA Spec 25.12.2.5 & 25.14.3).

2. Sprinkler QA checklist.

3. Certified sprinkler design, reviewed by FEMA.

Interior doors are not undercut.

Package System MHU's - Celing registers are 8" x 8" with damper.

Fire Extinguisher - Located on right side exterior wall adjacent to exterior entry door. Fire Extinguisher bracket shall be secured to wall studs.

Fire Extinguisher - UL listed with a minimum rating of 1A;10:B:C.

UFAS - Fire extinguisher must not protrude more than 4" from wall surface and top of fire extinguisher cannot be 48" above the floor surface.

Floor

Continous one piece vinyl flooring covering. No cuts or gouges. All repairs must be approved by FEMA or their designee.

Wall / Ceiling

Electrical

MHU Contact Placard has white background, black 1” font stating “FEMA

Owned & Maintained Unit Contact FEMA at 800-621-FEMA”, with space to incorporate the unit's barcode.

UFAS - 30" x 48" clear floor space in front of each appliance.

Wall finish - Off white color with egg shell finish.

General

Wall joints door/window frames and corners (visble or not) are finished

(covered with matching molding).

All trim molding is color matched/coordinated to the interior wall finish.

Trim and wall panels are caulked.

Interior ceiling height is seven feet, six inches (7' - 6").

UFAS - Accessible route requirement: mimimum 36" width and a clear headroom of 80" above finished floor. Must connect entry to the living spaces, kitchen, accessible bathroom, accessible bedroom.

Full length mini-blinds located at each window.

Access panels for bathrooms and washer/dryer area shall be purchased access panels, similar to Oatley 34056 or equivalent. Gypsum with permanently attached non-plastic edging is not acceptable.

INTERIOR

Exterior entry doors have a 30” x 15” interior door mat.

MHU Contact Placard is affixed to the interior side of exterior doors, adjacent to upper peephole. Top of sign not higher than approxomately

66” above finished floor.

Kitchen

1/2" x 2" door and window frame molding.

1/2" x 2" crown and baseboard molding.

Door stops installed on door or wall at entry and closet doors. Door stop finish to match interior hardware.

Sprinklers installed in all rooms, including the water heater and TPS compartment, per certified sprinkler design.

All FEMA labels, as described in the FEMA specifications are installed.

The following paperwork shall be in the MHU document holder:

1. Completed Duct Leakage Test - Signed & Dated (FEMA Spec. 26.4).

2. Vendor Installation Manual.

3. Transportation Test Report completed, dated and initialed by FEMA inspector (FEMA Spec. 29).

100% of the refrigerator space below 54" from the finished floor and 3) refrigerator and freezer controls below 54" from the finished floor.

UFAS - Pantry has adjustable 3/4" thick shelves. 50% of the shelves below 48" from the finished floor.

8' Wide MHU - UFAS - Depth of range from wall cannot exceed 28".

14' Wide MHU - Minimum countertop continuous unobstructed space is

36”.

UFAS - All switches mounted at 44" minimum to 48" maximum from centerline of switch to fininshed floor.

8' Wide MHU - 24" minimum length of base cabinet.

Refrigerator is secured to floor at the front and to at least one wall stud behind it.

UFAS - Wall duplex receptacles mounted at 18" to the centerline of receptacle screw to finished floor. Receptacles shall be spaced no more that 6' from door opening and 12' between receptacles.

One dual telephone jack outlet shall be mounted on the wall at 18" to the centerline of receptacle to finished floor.

Electrical

Microwave minimum size is 1.2 cubic foot with child lock capable.

Located in Bedroom 1 closet for shipping purposes.

UFAS - Over the range combination microwave/exhaust hood is not acceptable.

Secondary shut off valves (one on the hot line and one on the cold line) shall be provided.

8 Wide MHU - Single bowl sink shall be 1) stainless steel, 2) self-rimming and 3) with faucet and spray nozzle 24" inches in length.

The external tank and pump system (TPS) will be delivered and installed at the MHU site by FEMA’s sprinkler vendor. The TPS alarm and annunciator panel are located on the external TPS. There is no TPS unit inside the 8 Wide Express MHU.

All specialty tools (e.g., sprinkler head or soffit installation/removal tool) for each MHU required to install, operate, maintain and decommission the sprinkler system. Tools shall be included in the red box located in the water heater closet that contains the sprinkler heads.
