About this file
This document is a Quality Assurance Surveillance Plan (QASP) Matrix that establishes performance measurement standards for contract execution. The matrix defines five primary performance elements with corresponding requirements, surveillance methods, monitoring frequencies, and acceptable quality levels.
Contract Deliverables require submission of significant work accomplished, issues, and planned work on a monthly basis through COR inspection, with an acceptable quality level of 100% on-time reports. Quarterly Contract Performance requires monthly reporting of labor hours to the Contracting Officer's Representative (COR), also with 100% on-time delivery as the standard. Overall Contract Performance is assessed through COR observation documented no later than 30 days after contract completion, requiring >90% of all performance elements rated as Satisfactory or higher with 100% accuracy. Regulatory Compliance demands annual reporting of labor hours pursuant to Service Contract Act (SCA) requirements and compliance with FAR Clause 52.222-50 (human trafficking provisions), with 100% compliance required. Invoicing requires monthly invoices submitted per contract procedures that are timely and accurate, reviewed and accepted monthly with 100% accuracy as the standard. The matrix concludes that performance meeting acceptable levels is considered satisfactory; performance below acceptable levels may result in an unsatisfactory overall performance rating.
