mailto:christopher.e.snively.mil@us.navy.mil mailto:pedro.h.deoliveira.mil@us.navy.mil

When available, the Government will furnish reasonable amounts of the following utilities for the work to be performed under this contract at no cost to the contractor. Information concerning the location of existing outlets may be secured from the Officer In Charge. The contractor shall provide and maintain the necessary service lines from existing Government outlets to the site of work.

3. WARRANTY

All materials, parts and workmanship shall be warranted against defects for a period of one (1) year from the date of acceptance.

Provide copies of any extended warranties specific to materials supplied.

4. SECURITY

4.1 Contractor personnel employed on the Station shall become familiar with and obey Station regulations, shall keep within the limits of the work (and avenues of ingress and egress), and shall not enter any restricted areas unless required to do so and are cleared for such entry. Contractor equipment shall be conspicuously marked for identification.

4.2 Naval Station Guantanamo Bay (NGSB) Base Access

(This is intended for informational purposes. Points of Contact are provided to further assist.)

Access to NSGB involves two things: clearance with NSGB Security (via submission and approval of the SECNAV 5512 form [attachments SECNAV 5512 FORM and NSGBINST 5530.4G SECURITY QUESTIONNAIRE]) AND coordination with Air Mobility Command (AMC) for authorization and reservation of flights (via submission of Letter of Authorization (LOA) [attachment LETTER OF AUTHORIZATION] and AMC Flight Reservation Request within attachment AMC TRAVEL PROCEDURES). To gain access to NSGB, contractors need the following:

1. A LOA signed by the Contracting Officer (KO) (attachment LETTER OF AUTHORIZATION). *KO will need to sign the LOA with the contractors information (contact, billing address, number). Contractors will then need to send the signed LOA with the AMC Flight Reservation Request when reserving flights.

2. Contractors should complete the SECNAV 5512 Form (attachment SECNAV 5512 FORM) and NSGBINST 5530 (attachment NSGBINST 5530.4G SECURITY QUESTIONNAIRE). Forms should be submitted to NSGB Security for clearance at NSGB_Entry_Clearance@us.navy.mil.

*The SECNAV form must include a hotel reservation/confirmation number or housing approval stamp in box 30 (next to signature) as proof of lodging. The form must also include a "sponsor" which would be the customer POC located at NSGB.

Sponsor will be provided to awardee.

***The forms take at least a few days for NSGB security to run background checks and process. They advise submitting at least 30 days before planned visit.

3. Contractors shall read the AMC Contractor Manual (attachment AMC TRAVEL PROCEDURES) and make reservations accordingly. The manual instructs contractors to submit the LOA, approved SECNAV 5512, NSGBINST 5530, and AMC reservation request (page 6-2 of the manual) as provided in the manual to kathleen.j.ordway.civ@us.navy.mil. She will then assist in coordinating flights.

*AMC flight schedules, flight check-in information, and baggage requirements are available at the following link:

mailto:NSGB_Entry_Clearance@us.navy.mil mailto:NSGB_Entry_Clearance@us.navy.mil https://www.amc.af.mil/AMC-Travel-Site/Terminals/CONUS-Terminals/NAS-Jacksonville-Passenger-

Terminal//

Below are details for lodging and rental car reservations at NSGB:

a. Lodging: Navy Gateway Inns and Suites is (757)458-2040 or 2044

b. Rental cars: (757)458-4030

For contractors to travel on AMC aircraft, the following is required:

a. Valid passport. Passport card is NOT valid for air travel.

b. Properly executed SECNAVFORM 5512 and NSGBINST 5530.

c. Letter of Authorization signed by the Contracting Officer (KO).

d. Form of payment or can be billed to their company if necessary. Flight costs are currently $393.00 each way (subject to change).

e. Baggage within allowable size/weight limits: 62 linear inches and 70LB.

NOTE: Plane departs NASJAX but returns to Jacksonville International.

NOTE: All passengers must report to the NASJAX AMC Terminal (BLDG 279) in accordance with AMC procedures. Early check-in for the rotator flight to NSGB will be from 0930-1530 the day prior to the flight departure.

4.3 Defense Biometric Identification System (DBIDS)

DBIDS increases installation security and communications by receiving frequent database updates on changes to personnel /credential status, law enforcement warrants, lost/stolen cards, and force protection conditions. The system provides a continuous vetting anytime the DBIDS card is scanned at an installation entry point.

If you currently have a Navy Commercial Access Control System (NCACS) card, the following is required to get a DBIDS credential:

Present your NCACS Card and a completed copy of the SECNAV FORM 5512/1 to the base Visitor Control Center representative.

The VCC will pull up your information in the computer, ensuring all information is current and correct.

Once your information is validated, a temporary DBIDS credential is provided.

Your temporary credential will have an expiration date, prior to which you will need to obtain your permanent DBIDS credential (~ 180 days).

For each additional U.S. Navy installation to which you need access, the first time you visit you only need to bring your DBIDS credential and statement of purpose for base access when arriving at the Visitor Control Center.

The representative will enter base access authorization and then you may proceed to work.

If you do NOT have an NCACS Card, the following is required to obtain a DBIDS credential:

Present a letter or official document from my government sponsoring organization that provides the purpose for your access.

Present valid identification, such as a passport or Real ID Act-compliant state driver's license.

Present a completed copy of the SECNAV 5512/1 form to obtain your background check.

Upon completion of the background check, the Visitor Control Center representative will complete the DBIDS enrollment process, which includes your photo, finger prints, base restrictions, and several other assessments; after all this is done, you will be provided with your new DBIDS credential.

You may now proceed to work.
