About this file

This is a Request for Proposal (RFP) application instructions document issued by the Utah Department of Health and Human Services (DHHS) for the Community-Based Peer Support Program (DHHS26131). The solicitation seeks proposals from emerging organizations to provide mental health prevention and treatment services in areas designated as mental health provider shortage areas by the Health Resources Services Administration. Applicants must demonstrate that at least 60% of their service population will seek mental health prevention, treatment, and recovery services. The proposal submission requires completion of multiple forms including IRS Form W-9, conflict of interest disclosures, and a budget utilization form, along with a structured narrative addressing mandatory minimum requirements and scorable technical criteria. The application instructions do not specify response dates, award dates, or contract term information.

The evaluation criteria include mandatory minimum requirements that restrict eligibility to emerging organizations with no more than 10 full-time employees that currently engage with mental health or substance use recovery communities through peer support services. Four allowable activities are specified with varying point values: hiring at least one full-time peer support specialist (2 points), providing direct peer support services (6 points), providing supervisor training to peer support specialists (8 points), or establishing a family-run organization within Utah (10 points). Applicants must also demonstrate plans for peer support specialist engagement in the Peer Support Steering Committee or Utah Behavioral Health Advisory Council and participation in technical assistance from national substance use or mental health agencies. Budget justification is required and weighted at 10 points in the evaluation process. The document does not specify funding amounts, contract duration, renewal options, or information regarding incumbents.
