Summary of Attachment B: Scope of Work CIVITAS Fungicide

This is a Scope of Work document prepared by the State of Utah Division of Parks and Recreation, in coordination with the State of Utah Division of Purchasing, outlining requirements for a fungicide supply contract. The project involves the supply and delivery of CIVITAS fungicide in three container sizes (2.5-gallon, 55-gallon, and 300-gallon totes) to four state golf course locations: Green River State Park Golf Course, Palisade State Park Golf Course, Soldier Hollow State Park Golf Course, and Wasatch Mountain State Park Golf Course. The estimated annual requirement is 400 to 500 gallons. The awarded vendor will serve as a secondary provider, meaning golf course personnel will first contact the primary contractor for fungicide; only if the primary contractor cannot fulfill the desired quantities will golf course personnel contact the secondary vendor. The contract term is five years, with no guarantee of fungicide usage under this agreement.

Vendor pricing must be guaranteed for one year from contract award, with all prices including delivery costs. After the initial guarantee period, vendors may request price adjustments with at least 30 days' notice and supporting market analysis documentation; any approved adjustment must be approved by the Director of the Division of Purchasing and will be guaranteed for the same length of time as the original pricing. The State will receive immediate benefit of any decreases in market prices or allowable discounts. This is anticipated to result in a single contract award to the lowest cost responsive and responsible bidder. No mandatory site visit is required prior to bidding, though bidders are strongly encouraged to inspect the delivery locations to assess conditions affecting performance costs.

Attachment B: Scope of Work and Pricing

Scope of Work

The Utah State Division of Parks and Recreation is acquiring services from a vendor to provide fungicide and deliver to four separate locations. Vendor shall provide CIVITAS fungicide (estimated 400-500 gallons per calendar year) to the State of Utah Division of Parks and Recreation golf courses listed below, in the following container sizes: 300-gallon tote, 55 gallon, and 2.5 gallon (CIVITAS One Premix)

Vendor will be a secondary provider for the fungicide. Actual order quantities may fluctuate: Golf course personnel will contact the Primary Contractor first to provide desired quantities. If the Primary Contractor is unable to provide the desired quantities, golf course personnel may then contact awarded vendor to provide CIVITAS. There is no guarantee of use on this contract.

Vendor must be able to deliver to each of the State of Utah Division of Parks and Recreation Golf Courses at the following addresses:

• Green River State Park Golf Course – 550 Green River Blvd, Green River, UT 84525

• Palisade State Park Golf Course – 2200 East Palisade Road, Sterling, UT 84665

• Soldier Hollow State Park Golf Course – 1370 West Soldier Hollow Ln, Midway, UT 84049

• Wasatch Mountain State Park Golf Course – 12810 Warm Springs Rd, Midway, UT 84049

Pricing

Vendor guarantees its pricing for one (1) year. After the guarantee period, any requests for price adjustment must be made at least thirty (30) days prior to the effective date. A request for price adjustment must include sufficient documentation (market analysis) supporting the request. Any price adjustment will not be effective unless approved by the Director of the Division of Purchasing. A price adjustment will be guaranteed for the same length of time as the original price guarantee. The conducting procurement unit will be given the immediate benefit of any decrease in the market, or allowable discount.

Prices must include delivery.
