About this file

This is an Anti-Human Trafficking Affidavit form required by Broward County, Florida for vendors responding to a solicitation for Plantronics and JPL headsets and accessories. The affidavit must be completed and submitted with the vendor's bid response or within three business days of the County's request. Failure to submit the form within the specified timeframe may result in the vendor being deemed nonresponsive. The form certifies that the vendor does not use coercion for labor or services as defined in Section 787.06, Florida Statutes, and requires the vendor's authorized representative to sign the affidavit under penalties of perjury, providing their printed name, title, and date of execution.

The affidavit applies to the Plantronics Headsets and Accessories contract, an open-end agreement for supplying headsets and accessories to Broward County's Office of Regional Communications and Technology (ORCAT), 911 Public Safety Answering Points (PSAPs), and other County agencies. Vendors must be authorized resellers for both Plantronics and JPL products to be eligible. All solicitation responses must be submitted electronically through BPRO by the specified closing date and time, with vendors required to obtain a submission receipt confirming receipt. The County encourages vendors to submit responses in advance of the deadline and provides a specified question period for vendors to seek clarification on solicitation requirements through BPRO.
