About this file

This document is an Acknowledgements & Preference Application form for a Louisville Metro Government contract opportunity. The form allows bidders to acknowledge receipt of addenda, certify minimum wage standards, claim local vendor preference, and indicate participation in an apprenticeship program.

The document outlines three key preference certifications: a minimum wage preference requiring all full-time employees be paid at least $10.10 per hour, a local vendor preference for businesses meeting specific designation requirements, and an apprenticeship program preference for construction contracts exceeding $250,000. Each preference comes with specific acknowledgments and potential financial penalties for providing false information, including fines of 30% of the contract amount for minimum wage misrepresentation and 25% of the total contract for improper local vendor preference claims.
