PROCUREMENT (“API”s)

The API(s) approved for this project are selected below by ☒. Any bid that fails to comply with the API requirements after the period allowed for waiver requests has lapsed shall be deemed non-responsive. Fillable pdfs of all EBO forms can be found on the OEBO website at http://discover.pbcgov.org/oebo/Pages/Compliance-Programs.aspx. Also, see the EBO Ordinance and

Countywide PPM CW-O-043 for further information on APIs.

☐ Waiver

The Office of EBO has granted a waiver of the EBO Program API(s) for this Work Order.

☐ SBE Sheltered Market for Small Construction Contracts

Under this contract, Work Orders $100,000 and below are reserved for competition among only certified SBEs. Small prime construction contracts (single trade or multi-trade) valued at or below $100,000 may be reserved for sheltered market competition where only certified SBEs are eligible to submit bids or quotes.

☐ SBE Subcontracting Program

A minimum mandatory goal of 20% SBE participation is established for this contract. The EBO Office shall reduce or waive this goal when there is inadequate availability of SBE prime and / or subcontractor firms. Respondents/bidders may request such waivers at least seven (7) days prior to bid opening based on submission of adequate Good Faith Efforts documentation.

☒ SBE Price Preference For Single Trade Construction

The work orders under this contract shall be awarded to the lowest, responsive, responsible bidder unless a certified SBE’s bid is within ten percent (10%) of the lowest non-SBE bid, in which case the award shall be made to the certified SBE bidder. Construction contracts where there are no opportunities for subcontracting (i.e. single trade), may include a provision requiring awards of the contract to be made to the lowest responsive, responsible respondent or bidder unless a certified SBE’s bid is within ten (10) percent of the lowest non-small business bid, in which case the award shall be made to the certified small business bidder submitting the lowest responsive, responsible bid at the price that it bid.

☐ M/WBE Subcontracting Goal*

_______% (Up to 40%) of this Contract shall be subcontracted to certified M/WBEs owned by African

American, Hispanic American, Asian American, Native American, or non-minority women persons.

The EBO Office shall reduce or waive this goal when there is inadequate availability of M/WBE prime and

/ or subcontractor firms. Respondents/bidders may request such waivers at least seven (7) days prior to bid opening based on submission of adequate Good Faith Efforts documentation. In the absence of a waiver granted by the Office of EBO, a respondent/bidder who does not commit to satisfying the M/WBE subcontracting goal shall be considered non-Responsive.

☐ M/WBE Segmented Subcontracting Goals*

___________% of the M/WBE subcontracting goal shall be achieved through the utilization of certified M/WBEs owned by ☐ African American, ☐ Hispanic American, ☐ Asian American, ☐ http://discover.pbcgov.org/oebo/Pages/Compliance-Programs.aspx

Native American, and ☐ non-minority women persons (check applicable). M/WBE Segmented

Subcontracting Goals are established on an individual County contract wherein an overall combined

M/WBE goal is accompanied by subsets of one or more smaller goals that specifically target the participation of a particular segment of Minority Group Member segments or the WBE segment based upon that segment’s relative availability. Such segmented goals shall specifically target the participation of a particular segment of business enterprises owned and controlled by women or certain Minority Group

Members (e.g., African-Americans, Hispanic-Americans, Asian-Americans, or Native Americans) based upon relative availability, as well as the existence of consistently and significantly greater patterns of underutilization and disparity within an industry as compared to other gender and Minority Group Member categories of M/WBEs. (For example, if an overall M/WBE subcontracting goal is set at 38% on a given contract, the segmented subcontracting goal may require that at least 23% of that 38% shall be satisfied through the utilization of African American and Hispanic subcontractors.) The EBO Office shall reduce or waive this goal when there is inadequate availability of M/WBE prime and / or subcontractor firms.

Respondents/bidders may request such waivers at least seven (7) days prior to bid opening based on submission of adequate Good Faith Efforts documentation. In the absence of a waiver granted by the

Office of EBO, a respondent/bidder who does not commit to satisfying the M/WBE segmented subcontracting goals shall be considered non-Responsive.

☐ M/WBE Joint Venture/Partnership/Teaming Incentive for Contracts greater than $2.5 million

_______ (Up to 20%) evaluation points out of 100 are reserved for qualifying M/WBE joint ventures.

See, Exhibit “Z” for joint venture/partnership teaming incentive program requirements. Incentives are established to promote joint ventures, partnerships, or teaming arrangements between larger established firms and M/WBE firms, or between and among SBE and M/WBE firms. For RFPs, the incentive may be for up to twenty percent (20%) of one hundred evaluation points to be reserved for qualifying M/WBE joint ventures where the certified M/WBE joint venture partner owns 50% or greater, and performs 50% or greater of the work, of the overall joint venture. Proportionately fewer evaluation preference points would be awarded to the joint venture based upon lesser percentages of ownership by the M/WBE partner.

☐ M/WBE Evaluation Preference for RFPs for Prime M/WBE firms

_________ (Up to 15%) of the evaluation points are reserved as a preference for proposals submitted by certified M/WBE firms. Evaluation panels shall assign point preferences equal to up to 15% of the total points assigned for the evaluation, scoring and ranking of construction-related proposals submitted by those certified M/WBE firms. An M/WBE awarded a prime contract may not subcontract more than 49% of the contract value to a non-M/WBE firm.

SYSTEM NOTESSYSTEM NOTESSYSTEM NOTESSYSTEM NOTES

This quotation is based on email received 12/14/17 from Steve Fischer.

Request included CAD line diagram, however no further information was provided Therefore:

A. Crestron is not responsible for perceived misinterpretation of room/equipment quantities OR make/models of equipment being used;

B. This is a preliminary budgetary quote for reference only, and the equipment list shown may not constitute a complete working system design needed as per the customer’s project requirements;b C. Prior to using this quotation as the basis for a bid or placing an order, the Dealer must verify all equipment to ensure that Crestron design matches dealer’s design intent;

D. Please submit complete system specifications, drawings and functional descriptions to Crestron Sales Support Services for a revised proposal.b

Purchase-Orders placed on behalf of this Proposal MUST reference the above Crestron Quote-Number:

• Please send your PO to orders@crestron.com

• For Government Partner related projects, please send your PO to gov.orders@crestron.com to automatically receive the benefits associated with the Crestron GSS Program.

This Proposal has been released under the assumption that the following information has been provided to Crestron:

• A detailed Scope of work defining system operation, macros, page-flips, individual device functionality, and Room-Scheduling or Fusion integration requirements and functionality;

• Final/approved/complete System Design Specifications and AV/Control-Flow Drawings;

• Final/approved User-Interface (touchscreens, keypads, etc.) layouts and associated functional flow-diagrams;

• Final/approved list of make/models/quantities of equipment and rooms to be controlled;

All Crestron hardware is quoted separately from Crestron purchasable software and licensing (i.e. Room-Scheduling, Fusion, etc.). If both Crestron hardware and software/licensing is required for your project, please ensure that you obtain separate quotes for each.

Prior to using this Proposal as the basis for placing a Bid or an Order, the Dealer must verify that all required equipment or software/licenses are included, and that the bill of materials shown is complete and in compliance with the design intent and functional needs of the project.

To the extent that any of the above information was not provided by the time this Proposal was released by Crestron, it should then be considered as a preliminary/budgetary pricing-quote for reference only, and that the equipment or software licensing shown may not constitute a complete working system that is compatible with the project requirements and customer’s expectations. Also, Crestron reserves the right to revise the quoted pricing in order to cover fees for additional equipment, software/licensing, and/or services that may be needed.

Placing a Purchase-Order on behalf of this Proposal serves as acknowledgement by the Dealer that the bill of materials quoted is accurate and complete.

Unless specifically stated otherwise, all IT/Network switches, power-supplies, wiring, wireless interfaces, subnetworks, and configurations are the responsibility of the dealer in coordination with the end-customer. Also, configuration of network hardware/software for the purpose of integrating with Crestron system hardware and Room-Scheduling or Fusion Software/Servers must adhere to the recommended best practices as outlined in all corresponding Crestron product manuals, Design-Guides, and other relevant supporting documentation available on the Crestron Website.

Products within this quote may be part of the Autodesk SEEK Revit database website, and can be specified in designs for Building Information Modeling (BIM). Various Crestron products and system application diagrams are available on the Crestron website in both Revit and ACAD file formats.

This proposal contains legally privileged and confidential information intended solely for the use of the addressee. If you are not the intended recipient, you are hereby notified that any reading, dissemination, distribution, copying, or other use of this message or its attachments is strictly prohibited. This information remains property of Crestron Electronics, Inc., and is not to be distributed outside your company without expressed written permission from Crestron.

DM System quoted may include some 4K video capability. For complete 4K system compatibility, or device specific 4K Support, additional equipment may be required;

please visit www.crestron.com/4k for a list of Crestron 4K Certified devices or contact Sales Support Services for a detailed design review.

Please note the wire lengths for 4k resolutions are as follows:

DM-CBL-ULTRA DigitalMedia Ultra Cable (uses DM-CONN-20 connectors)

• 330 ft (100 m) for 4K Ultra HD

DM-CBL-8G DigitalMedia 8G™ Cable (uses DM-8G-CONN-100 connectors & DM-8G-CRIMP)

• 230 ft (70 m) for 2560x1440 WQHD

• 230 ft (70 m) for 2560x1600 WQXGA

• 230 ft (70 m) for 4K Ultra HD

CAT5e (or better) UTP or STP

• 165 ft (50 m) for 2560x1440 WQHD

• 165 ft (50 m) for 2560x1600 WQXGA

• 165 ft (50 m) for 4K Ultra HD Due to the complex technology involved in digital video transmission, as well as the lack of standardization between manufacturers, we have discovered that some third-party 4K digital video distribution devices (switchers/transmitters/receivers/amplifiers/etc.) use proprietary HDCP Key (KSV) management techniques, non-standard CEC transmissions, insufficient EDID/display-resolution support, and/or do not adhere to the HDMI/HDCP protocols. These issues may result in improper system operation beyond the control of the Crestron/DM system. Please see www.crestron.com/4k for a list of Crestron 4K Certified Devices. Crestron cannot guarantee compatibility with 4K devices which have not been certified by Crestron.

This system design contains a mix of 2K and 4K digital video sources. Please be aware that 4K and other resolutions with a pixel clock of greater than 165MHz can only be input into the Crestron product that has 4K compatibility and can be routed 4K compatible destinations. Please contact Sales Support Services for additional information on 4K compatible product within this design.

System quoted at "LIST" Pricing. Possession of this quote does not constitute authorization for direct purchase from Crestron. Please contact your regional representative for dealership inquiries.

Crestron speakers have been included within this quote. Additional speaker mounting hardware is available for an additional charge.
