About this file
This is Amendment #1 to a Request for Proposal (RFP) issued by the University of Wisconsin-Madison for three blood gas analyzers to be used in veterinary care. The amendment was issued on December 7, 2018, with a proposal due date of December 19, 2018, at 2:00 PM CST. The amendment updates Section 4 (Requirements and Specifications) and corrects the Cost Proposal Excel spreadsheet based on questions received from bidders. Key specifications require analyzers to measure pH, PCO2, PO2, SO2%, hematocrit, hemoglobin, sodium, potassium, chloride, ionized calcium, glucose, BUN/urea, lactate, and optionally ionized magnesium or creatinine. Analyzers must be validated for veterinary patients, include built-in printers, be capable of stat testing with recovery within 10 minutes after control cartridge replacement, include co-oximetry capability, and support networking of all three units with a shared database. Installation, training for up to six individuals, and delivery coordination must be provided by the contractor. Proposal prices and terms must remain firm for 60 days from proposal submission to opening. The University uses the Advanced Technology Corp LIS called VADDS with Midas middleware and belongs to Vizient Group Purchasing Organization.
The RFP requests pricing through three distinct lot options: Lot 1 involves outright purchase of three analyzers with one-year warranty and separate annual service pricing for years two through five; Lot 2 provides cost-per-sample pricing with contractor retaining equipment ownership at the five-year term conclusion; and Lot 3 provides cost-per-sample pricing with University ownership of equipment at contract termination. All consumables costs must be capped for annual increases over the five-year contract term. The pricing baseline assumes 3,000 samples annually across all three analyzers, with tiered pricing requested for volumes between 3,001 and 5,000 samples. Optional pricing is requested for a fourth instrument purchase in year three and for co-oximeter consumables covering 250 patient samples annually. Reagent cartridges must have minimum six-month shelf life at room temperature, though 18 months is preferred. All service must include on-site support with same or next-day repair, parts and labor coverage, annual preventative maintenance, and unlimited telephone support for the contract term.

QUESTIONS AND ANSWERS
Q1: Section 3.17 PERFORMANCE MEETINGS What are the recommended time frames/range for the performance meetings? Are Call in's via the telephone by our Clinical Team acceptable? In most cases quarterly to semi-annual business reviews are acceptable.
A1: Quarterly performance meetings in person or by phone are acceptable.
Q2: Section 4.1.1 SPECIFICATIONS In reference to analyzers being validated for Veterinary use, will an "OFF LABEL" and or for "RESEARCH ONLY" recommendation be acceptable?
A2: The University will consider OFF LABEL and RESEARCH ONLY analyzers. Since this is not a mandatory requirement it will be scored during the technical evaluation. See revised Section 4, # 4.1.1.
Q3: Section 4.3.3 REAGENTS Our Cartridge pack has a six months shelf life and does not require refrigeration. Is the recommended 18 months room temperature dating, a requirement or recommendation?
A3: This is not a mandatory requirement and will be scored during the technical evaluation.
Q4: ATTACHMENT B: COST PROPOSAL FORM
On the cost proposal it states to list the quantities needed to perform 3,000 samples per annum. Is this 3,000 samples on a per analyzer basis, or is the 3,000 samples the total number of samples to be run across all analyzers?
A4: The 3,000 samples per year is over the three analyzers and not per analyzer.
Q5: EXPECTED VOLUME
Will you be able to provide us with a historic utilization/volume? What is future expected volume and will it be consistent over time?
A5: The number of tests provided in the last 12 months was 2,680 and the previous 12 months was 2,840. Our Cost Proposal is based on 3,000 samples annually. We expect increased samples during the contract term once the new facility opens. The Cost Proposal Form requests pricing for samples in excess of 3,000 annually for Lots 2 and 3.
| Q6: | Does UW belong to a Group Purchasing Organization? |
| A6: | The University belongs to Vizient. |
| Q7: | What LIS do you use? |
| A7: | The LIS we use is through Advanced Technology Corp and it is called VADDS. The middleware is also from them and called Midas. Link to their site: https://vetstar.com/. See new specification in Section 4, # 4.2.6. |
