NATIONAL LONGITUDINAL SURVEYS (NLS) QUALITY ASSURANCE SURVEILLANCE PLAN

For: National Longitudinal Surveys of Youth: NLSY79 and NLSY97

Contract Number: < upon award, enter contract number>

Contract Description: The Contractor shall provide oversight and support for the NLSY79 and NLSY97 surveys. Services shall include survey design, mixed mode data collection, data processing, and data dissemination.

1. Purpose.

This Quality Assurance Surveillance Plan (QASP) provides a systematic method to evaluate performance for the stated contract. This QASP explains the following:

· What will be monitored?

· How monitoring will take place?

· Who will conduct the monitoring?

· How monitoring efforts and results will be documented?

This QASP is a “living document” and the Government may review and revise it on a regular basis. However, the Government shall coordinate changes with the contractor. Updates shall ensure that the QASP remains a valid, useful, and enforceable document. Copies of the original QASP and revisions shall be provided to the contractor and Government officials implementing surveillance activities.

The following FAR clause applies:

2. Government Roles and Responsibilities.

The following personnel shall oversee and coordinate surveillance activities.

a. Contracting Officer (CO) - The CO shall ensure performance of all necessary actions for effective contracting, ensure compliance with the contract terms, and shall safeguard the interests of the United States in the contractual relationship. The CO shall also assure that the contractor receives impartial, fair, and equitable treatment under this contract. The CO is ultimately responsible for the final determination of the adequacy of the contractor’s performance.

b. Contracting Officer’s Representative (COR) - The COR is responsible for technical administration of the contract and shall assure proper Government surveillance of the contractor’s performance. The COR shall keep a quality assurance file. At the conclusion of the contract or when requested by the CO, the COR shall provide documentation to the CO. The COR is not empowered to make any contractual commitments or to authorize any contractual changes on the Government’s behalf. The contractor shall refer any changes they deem may affect contract price, terms, or conditions to the CO for action.

4. Performance Standards.

Performance standards define desired services. The Government performs surveillance to determine if the contractor exceeds, meets or does not meet these standards. The Performance Requirements Summary Matrix, NLS Technical Exhibit 1 in the Performance Work Statement includes performance standards. The Government shall use these standards to determine contractor Performance Objective (The Service required—usually a shall statement)

Method of Surveillance

PRS #

Task 1: Management

PRS # 1.1.1

The Contractor shall submit a Management and Staffing Plan.

Plan shall be provided to the COR within 45 days business days of the contract award.

The Contractor shall submit a Document Management Plan (DMP).

Plan shall be provided to the COR within 45 days business days of the contract award.

The Contractor shall maintain an Electronic Document Management Repository (EDMR).

Document Repository shall be updated as needed. A list of documents added to the archive shall be submitted each month at the same time as the monthly progress report.

PRS # 1.2.3

The Contractor shall transfer or destroy all documents stored in the EDMR.

Upon expiration or termination of the contract, the Contractor shall follow the Agency’s direction as to the preservation, transfer, or destruction of all documents within the repository.

PRS # 1.3.1

The Contractor shall submit to NARA all Contractor Records to NARA

14 months after the end of fielding, Contractor will transfer to NARA all Contractor created NARA documents.

The Contractor shall submit a monthly progress report.

The Monthly Progress Report shall be submitted electronically on the

The Contractor shall submit Management call minutes from the prior calls.

PRS # 1.6.1

The Contractor shall submit potential agenda topics for the QMM.

Agenda topics shall be submitted to the COR 4 weeks prior to the QMM.

The Contractor shall provide meeting materials for the QMM.

Meeting materials shall be submitted to the COR 3 business days prior to the QMM.

The Contractor shall submit meeting minutes taken during the QMM.

Meeting minutes shall be submitted to the COR within 1 week after the QMM has taken place.

Task 2: Information Technology Security Requirements

PRS # 2.1.1

The contractor shall submit a Potential BLS Agent list.

Initial agent list shall be submitted within 30 days of contract award.

PRS # 2.1.2

The Contractor shall submit Training Certification reports.

Training Certification report shall be submitted once every Fiscal Year. Date of submission TBD.

PRS # 2.1.3

The Contractor shall submit BLS Agent Status Worksheets.

Agent Status worksheet shall be submitted quarterly or upon request of the ISSO or COR.

PRS # 2.1.4

The Contractor shall submit a Worksite Location List.

Worksite Location list shall be submitted within 30 days of the contract award, and with any proposed changes.

PRS # 2.2.1.1

The Contractor shall complete an intitial NLS System Assessment and Authorization if not already granted

The Contractor shall successfully complete a NLS System Assessment and Authorization (leading to Authority to Operate) within 4 months of contract award, if not already granted.
Upon completion of the NIST Security Controls Assessment Report, and acceptance or closure of any resulting POAMS, Authority to Operate status will be granted for 3 years.
The audit shall be performed by a third party auditor, contracted for by BLS.

PRS # 2.2.1.2

The Contractor shall assist NLS staff in creating the Assessment and Authorization Package.

After initial authorization, it shall be submitted every 3 years.
All documents shall contain information requested such that materials will pass review by BLS and/or an independent auditor.
Reviewed by NLS ISSO.

PRS # 2.2.2.1

The Contractor shall complete a NLS Partial System Self Audit.

During years there is not a full NIST Controls Audit, the contractor will show compliance on a subset of NIST controls selected by Department of Labor Office of Inspector General
Sufficient evidence is provided showing compliance with the selected NIST controls after review by BLS and POA&Ms have been submitted and accepted by BLS when compliance has not been proved.

PRS # 2.2.3.1

The Contractor shall complete a full NIST Security Controls Assessment audit.

Initial NIST Security Controls Assessment shall be completed within 4 months of contract award, unless previously authorized. After initial authorization, it shall be submitted every 3 years
Sufficient evidence is provided showing compliance with the selected NIST controls after review by BLS and an independent auditor and POA&Ms have been submitted and accepted by BLS when compliance has not been proved.

PRS #2.2.4.1

The contractor will use the template provided by NLS to record and update Plan of Action and Milestones.

Plan of Action and Milestones shall be submitted quarterly and updated as needed between quarterly submissions

The POA&M document shall include the specific deficiencies, the affected security controls, the necessary steps to complete the proposed solution or risk mitigation strategy, and the timeline.

PRS # 2.2.5.1

The Contractor shall submit a System Security Plan (SSP).

Initially submitted within 3 months of award. System Security Plan shall be submitted annually and updated as needed between annual submissions.

PRS # 2.2.6.1

The Contractor shall submit a Configuration Management Plan.

Configuration Management plan shall be submitted annually and updated as needed between annual submissions.

The configuration management plan will document the system boundary, detail what hardware and software tools will be used, and explain how change to the system is triggered, tracked and audited.

Reviewed by NLS ISSO.

PRS # 2.2.7.1

The Contractor shall submit a Business Impact Analysis.

Business impact analysis shall be submitted annually and updated as needed between annual submissions.
+/- 3 business days

The Business Analysis Plan will list system hardware and software, location, quantity, other resources needed to function, and links to other critical system resources.

Reviewed by NLS ISSO.

PRS # 2.2.8.1

The Contractor shall submit a Contingency Plan

Contingency Plan shall be submitted annually and updated as needed between annual submissions.
+/- 3 business days

The Contingency Plan will detail the NLS system disaster recovery tasks and procedures to be used when responding to any interruptions of NLS business operations. Contractor will take part in a simulation f an emergency designed to validate the viability of its IT plan and will detail the results of these tests.

Reviewed by NLS ISSO.

PRS # 2.2.8.2

The Contractor shall conduct Contingency Plan Testing following BLS instructions.

Contingency Plan Testing shall be submitted annually as directed and updated as needed between annual submissions.
+/- 3 business days

The Contingency Plan will detail the NLS system disaster recovery tasks and procedures to be used when responding to any interruptions of NLS business operations. Contractor will take part in a simulation f an emergency designed to validate the viability of its IT plan and will detail the results of these tests.

PRS # 2.2.9.1

The Contractor shall submit an Incident Response Plan.

Incident Response plan shall be submitted annually and updated as needed between annual submissions.

The incident response plan will detail how IT will detect, respond to and recover from network security incidents.

Reviewed by NLS ISSO.

PRS #2.3.1

The contractor will assist and participate in all Confidentiality and Security Compliance Reviews.

Confidentiality and Security Compliance Reviews shall be completed as needed, per the request of the NLS ISSO or COR.
Contract will assist as necessary with all audits, scans, and documentation and provide requested information in the time period allotted. Documents will pass review by BLS and/or an independent auditor

Task 3.

User Services

PRS # 3.0.1

The Contractor shall submit a user website maintenance and enhancement schedule.

The Website Maintenance and Enhancement Schedule shall be submitted within 4 months of the contract award. After the initial plan has been submitted, shall be updated annually.
+/- 5 business days

Reviewed by COR and the User Outreach team.

PRS #3.0.2

The Contractor shall maintain for BLS the Domain names of the NLS public use websites The contractor shall maintain for BLS the Domain names of the NLS public use websites, and shall keep these registries current.

Submit domain name documents annually

PRS #3.1.1

The contractors shall make available the Public use data shall be available for users on the internet both via the NLS Investigator extraction tool and in asci download

New public use data for a given round shall be added to the NLS database 14 months after the completion of the data collection.
+/ - 14 business days or as agreed upon by the COR.
Reviewed by NLS Research team.

PRS # 3.1.2

The Contractor shall submit a maintenance and improvement plan for the NLS Investigator data tool

NLS website Investigator data tool Maintenance and Enhancement Schedule shall be submitted within 6 months of the contract award. After the initial plan has been submitted, shall be updated annually.
+/ - 10 business days.
Review by COR and NLS designated staff.

PRS # 3.1.3

The Contractor shall make available all questionnaires and related survey documentation for prior and current rounds of data on the NLS user site.

Documentation associated with the public-use data release shall be made available on the NLS user site within 60 days of the release.
+/ - 14 business days or as agreed upon by the COR.
Reviewed by NLS Research team.

PRS #3.1.4.1

The Contractor will follow BLS Errata notification and remediation strategy

Upon discovery, the Contractor shall notify the NLS of Errata and their proposed remediation strategy and timeline within 1 week of identification.
+/ - 3 business days
Reviewed by the NLS Research team.

PRS #3.1.4.2

The contractor shall place notification of error description placed in errata section of the website

The Errata Page on the NLSInfo.org site shall be updated as needed, per the requirements of the remediation strategy and agreed upon by the COR.

PRS # 3.1.5.1

The Contractor shall update the Bibliography with new entries.

Bibliography updates shall be made quarterly or as needed.

The Contractor shall provide a listing of new Bibliography entries.

A listing of new bibliography entries shall be provided on a quarterly basis.

PRS # 3.2.1

The Contractor shall maintain and submit a listing of user questions and answers.

An updated list of User Questions shall be submitted each month in conjunction with the monthly progress report.

PRS # 3.3.1

The Contractor shall propose a list of professional conferences to attend in the coming year.

To promote NLS data and support user outreach, the Contractor shall propose a list of professional, technical or industry conferences where the NLS surveys shall be represented.
+/- 5 business days.
Reviewed by the COR.

PRS # 3.3.2

The Contractor shall propose and develop informational materials about the survey.

Informational materials about the NLS survey and its data shall be developed for use at conferences or other outreach activities. If materials that have not been approved are submitted in preparation for an up-coming conference, they must be provided at least 3 weeks in advance for review.
+/- 3 business days.

PRS # 3.4.1

The Contractor shall submit a cohort harmonization plan.

The Cohort Harmonization Plan shall include a list of potential variables to be included in the cohort comparability data set as part of the harmonization effort. The plan shall be due 4 months from the data of contract award.
+/- 5 business days.
Reviewed by COR and designated NLS staff.

PRS # 3.4.2

The Contractor shall create and add harmonization variables and documentation to the NLS website and data tool Investigator.

Harmonization variables and documentation shall be added to Investigator on a flow basis, per the Cohort Compatibility Plan.
+/- 5 business days.
Reviewed by COR and designated NLS staff.

Task 4.

Advisory Services

PRS # 4.1.1

The Contractor shall reserve hotel rooms for Technical Review Committee (TRC) members.

A block of hotel rooms shall be reserved for TRC members wishing to stay overnight. The reservations shall be made 2 months prior to the meeting date.
+/- 2 business day
Reviewed by NLS Director and COR.

PRS # 4.1.2

The Contractor shall arrange for a lunch for TRC members.

A catered lunch shall be arranged by the Contractor for members attending the biannual TRC meeting, no later than 2 weeks prior to the meeting date.

PRS # 4.1.3

The Contractor shall prepare materials for TRC meetings.

Draft materials for the biannual TRC meetings, such as handouts and supporting materials, shall be provided if requested by the Director. The Director shall make the request no later than 1 month prior to the meeting. The Contractor shall provide the draft materials no later than 2 weeks prior to the TRC meeting date.

PRS # 4.1.4

The Contractor shall record the minutes of the TRC meetings.

General minutes for the biannual TRC meetings shall be recorded and prepared by the Contractor. A draft shall be sent to the Director and COR no later than 2 weeks after the date of the TRC meeting.

PRS #4.1.5

Reimburse TRC members for expenses related to TRC meeting

Reimbursement for TRC member’s expenses shall be provided no later than 2 months after the biannual meeting.

PRS # 4.1.6

The Contractor shall provide suggestions for potential new TRC members

At the request of the Director, the Contractor shall provide a list of potential replacement candidates for retiring members. The Director may make this request at any point during the year. From the time of the request, the Contractor shall have 1.5 months to provide a list of viable candidates.

PRS # 4.2.1

The Contractor shall provide a list of potential candidates for NLSY79 and the NLSY97.

If requested by the Director, the Contractor shall provide a list of potential candidates for the role of Principle Investigator for the NLSY79 or the NLSY7. The list shall be due three months after the request, or as negotiated with the COR. Selection of the PIs shall be contingent upon the decision of the Director.

PRS # 4.2.2

The Principle Investigators shall advise the NLS design team on survey content.

The PIs shall provide feedback on survey content and methodology either through email or design meetings. Date at which feedback is due shall be determined in conjunction with the COR.
+/-5 business days, unless otherwise indicated by the COR.
Reviewed by the NLS design team.

PRS # 4.2.3

The Principle Investigators shall attend the QMM and TRC meetings.

The PIs shall attend the biannual in-person QMM and TRC meetings. If requested, they shall prepare a short summary on the status of their respective surveys, to be submitted to the Director two weeks prior to the TRC meeting.

PRS # 4.2.4

The Principle Investigators shall coordinate with external stakeholders on survey content and direction and provide a short summary of these activities

The PIs shall identify and coordinate with external survey stakeholders on the content and direction of the surveys. They shall provide a short summary of these activities at the biannual TRC meeting. Reports shall be submitted to the Director in conjunction with their survey status reports, two weeks prior to the TRC meeting.

PRS # 4.2.5

The Principle Investigators shall advise on how best to promote NLS data.

Each PI shall advise the BLS and Contract staff on how best to promote the use of NLS survey data. They shall provide written recommendations six months prior to the release of the latest round of their survey’s public use data.

Task 5. Respondent Outreach (Respondent Website)

PRS #5.1.1

Create and Maintain a website where respondents can update contact information and electronically request appointments

Task 6. NLSY97 Round 19 Data Processing

PRS #6.1

The Contractor shall create a Cleaned and Coded Data file.

Due date of the file shall be determined in conjunction with the COR.

The Contractor shall submit a Created Variable plan.

Due date of the file shall be determined in conjunction with the COR.

PRS #7.1.1

The contractor shall submit an Interim Contact Plan.

The interim contact plan shall be delivered no later than 3 months after the end of the field period for the previous round.

The contractor shall provide Interim Contact Respondent Materials for review.

Interim contact materials will be submitted as agreed upon in the Interim Contact Plan, at a date to be determined in conjunction with the COR.

PRS #8.1.1

The Contractor shall produce a Questionnaire Report assessing CAPI/CATI compatibility.
