Attachment J.H.4

QUALITY ASSURANCE SURVEILLANCE PLAN

1.0 SCOPE

The purpose of this surveillance plan is to provide the Bureau of Labor Statistics (BLS) with the procedures necessary to monitor the performance of Contract [Number to be Added]. It provides detailed methods for identifying, recording, and reporting performance deficiencies or accomplishments observed in the requirement.

2.0 FUNCTIONS SURVEYED

The functions surveyed will be the technical and timely performance of the contract. Overall performance on this contract will be reviewed bi-weekly between the Contracting Officer Representative (COR) and the Task Monitor (TM). This is a firm-fixed-price with optional CLINs contract.

3.0 PROCEDURES

3.1 DOCUMENTS/REPORTING

The contractor has the primary responsibility for ensuring the quality of their services in meeting the contract performance requirements. The contractor shall develop, implement and maintain a Quality Control Plan that describes the methods used to monitor and review performance to ensure it conforms to the contract performance requirements listed in Part 5 of the PWS. Such reviews are performed by the contractor in order to validate their operations and assure the Government that the services are in compliance with contract requirements. Any reports or results generated by the contractor’s Quality Control Plan shall be provided to the COR, as requested.

3.2 INSPECTION

Government contract quality assurance will be performed by the COR and TM who are responsible for monitoring and assessing contractor performance to ensure that services conform to contract performance requirements. The COR will assess contractor performance formally and informally on an ongoing basis throughout the life of the contract. To assist the COR, the TM assigned to the contract observes the work performed in cooperation with the contractor’s project director to ensure that work proceeds in accordance with the contract. The COR will be included in all written communication between the TM and contractor. Any observed contract performance issue(s) will be documented and reported to the COR. The COR will inform the contractor of any failure to comply with requirements, document contractor steps to correct the cause of the failure, and recommend that the CO take appropriate corrective action, when necessary.

3.2.1 SPECIFIC INSPECTION METHODS

The COR will be perform 100% inspection of resumes and deliverables for the Staffing, Quality Control, and Timeliness requirements by reviewing staff resumes, and reviewing deliverables listed in section 1.4 Tasks. The TM will assist the COR by reviewing key personnel resumes to ensure PWS qualifications are met, communicating with the contractor about technical issues and delays, and reviewing all deliverables for quality.

The COR will monitor Communication and Technical support based on feedback received from Task Monitor and BLS Agent feedback through email. The BLS may survey BLS agents about their experiences with the VDE.

The COR will review the monthly reports provided by the vendor to ensure access and removal performance requirements are met.

The COR will document when notice of scheduled downtime is provided and document the length of each scheduled downtime each quarter to ensure performance requirements are met.

The COR will document unscheduled downtime during each quarter to ensure performance requirements are met.

The Division of Management Services (DMS) will perform periodic surveillance remotely and on-site to ensure the security performance requirement is met and provide feedback to the COR.
