Amendment-000002:

Text1: Amendment 2:

1. Question: Would it be possible to get the equipment list broken down by site?

The only list we have is PIMC, San Carlos Service Unit, Whiteriver Service Unit and Hopi Service Unit. See attachment for Hopi Service Unit, San Carlos Service Unit, and Whiteriver Service Unit audiology equipment list. We are only giving estimated quantities because we don’t know the exact count.

Text2: 2. Change statement in Paragraph 6.b. Repair Service. Changes for Amendment-2 are in red font, see Attachment 16-247-SOL-00027 Amendment-2.

Statement:

From: As a part of services provided under this contract, at no additional cost to the Government, perform all repairs necessary to prevent a breakdown or failure of Government-owned equipment covered by this contract.

To: As a part of services provided under this contract, the contractor shall perform all repairs necessary to prevent a breakdown or failure of Government-owned equipment covered by this contract.

3. Change Paragraph title and statement in Paragraph 6.c. Replacement Parts and Supplies. Changes for Amendment-2 are in red font, see Attachment 16-247-SOL-00027 Amendment-2.

Title:

From: 6.c. Replacement Parts and Supplies To: 6.c. Replacement Materials and Supplies

Statement:

From: All necessary repair parts, materials, removal, and disposal of bio hazardous materials (if applicable) for maintenance of the instruments listed herein shall be furnished at no additional cost to the Government.

To: All necessary materials, removal, and disposal of bio hazardous materials (if applicable) for maintenance of the instruments listed herein shall be furnished at no additional cost to the Government.

4. Question: Is there an expectation to supply parts such as speech monitor mic headsets that are common to fail or Immitance and OAE probes. Parts would be listed under the CLIN for repair services.

5. Question: With Indian Health Service Contracts is an additional CLIN line an option? There is a CLIN line for repair services that will cover parts.

6. Question: There are two sound booths within the equipment list, and the contract specifies for relocation. In the event the contract was awarded would relocation of sound booths need to be considered. In many cases this type of relocation requires a very specialized team can be very expensive. We would like to know more regarding if sound booths would be omitted from relocation. The sound booths are not moving and we will not amend the solicitation.

7. Question: If we were unable to respond onsite within 48 Hours for a service call would shipment of the necessary part until we are able to arrive be an acceptable resolution. In many cases sourcing a part if it is not common can take up to 3 weeks. The 48 hours is to respond to the service call and you would need to communicate to the COR when the parts are going to shipped and installed.

8. Question: Would we be able to offer annual calibration services only and then take repair cases individually? Yes we added a CLIN for repair service for each option year.
