SECTION V – BASIS FOR AWARD
This acquisition will utilize Lowest Price Technically Acceptable (LPTA) source selection procedures in accordance with FAR 15.101-2, as supplemented. By submission of its offer, the Offeror accepts all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements. Failure to meet a requirement may result in an offer being determined technically unacceptable. Offerors must clearly identify any exception to the solicitation and conditions and provide complete accompanying rationale.
In order for an Offeror to be considered for award, the proposal must receive an “Acceptable” rating in every non-price factor. Any proposal receiving a rating of “Unacceptable” in any non-price factor will not be further evaluated.
The Government intends to award one (1) contract for this purchase request.
For the purpose of award, the government shall evaluate offers based on the evaluation factors described below:
EVALUATION FACTOR CRITERIA
FACTOR 1 - Technical (Acceptable/Unacceptable)
Offeror shall use the Statement of Work to understand the requirements of this service and to use their expertise and knowledge to provide a clear and concise explanation for the sub-factors listed under Technical.
The Government will evaluate the contractor’s proposed explanation of the sub-factors to ensure that it reflect sound understanding of Factor 1 – Technical.
Sub Factor 1: Contractors Qualifications
Provide statement of certifications to demonstrate proficiency of the technical staff in providing this type of service. Training and experience must be relevant for the specific types of equipment listed herein.
Sub Factor 2: Preventive Maintenance and Repair Plan
Provide a specific plan for providing preventive maintenance checklists, provide a system of records for PMs and Repairs services, and software updates/upgrades to maintain the equipment to the original manufacturer specifications (including updates).
FACTOR 2 – Past Performance Factor Confidence Assessment
The Government will conduct a performance evaluation based upon the past performance of the offerors performing major or critical aspects of the requirement as it relates to the probability of successfully performing the solicitation requirements.
The Government will consider each offeror’s demonstrated, current, relevant record of performance in supplying products and services as a prime contractor that meet the contract’s requirement. Contracts completed within the last 3 years are generally considered recent.
In conducting the past performance evaluation, the Government may use data provided by the offeror and data obtained from other sources it considers current and accurate, including the Past Performance Information Retrieval System (PPIRS).
Provide past/current performance information for those companies or installations where your firm has recently (during the past three years) provided similar services for the type instruments specified in the SOW. Provide the following information for each reference:
Name of Contracting Organization (Government or Non-Government) Contract Number or Account Number (for subcontracts, provide the prime contract number and the subcontract number) Contract Type Years of Service Total Contract Value Description of Requirement Contracting Officer’s or Point of Contact, Name and Telephone Number (The offeror may provide information on problems encountered on the identified contracts and the offeror’s corrective actions.)
FACTOR 3 - Price
The Government will evaluate the quote to determine the price fair and reasonableness in accordance with FAR 13.106-3(a).
Offer must be good for 30 calendar days after submission.
FOB Destination CONUS (Continental U.S.). Shipping charges shall be included in the purchase cost of the product.

TECHNICAL RATINGS
Rating Definition Acceptable The proposal clearly meets the minimum requirements of the solicitation.
Unacceptable The proposal does not clearly meet the minimum requirements of the solicitation.
Factor 2 – Past Performance Factor Confidence Assessment:
One of the following ratings will be assigned.
PAST PERFORMANCE RELEVANCY RATINGS
Rating Definition Very Relevant Present/past performance effort involved essentially the same magnitude of effort and complexities this solicitation requires.
Relevant Present/past performance effort involved much of the magnitude of effort and complexities this solicitation requires.
Somewhat Relevant
Present/past performance effort involved some of the magnitude of effort and complexities this solicitation requires.
Not Relevant Present/past performance effort did not involve any of the magnitude of effort and complexities this solicitation requires.
The combination of relevancy and recency, and performance documented by the information collected by the Government, will result in one of the following ratings:
PAST PERFORMANCE RATINGS
Rating Description Acceptable Based on the offeror’s performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror’s performance record is unknown (See note below.).
No Confidence Based on the offeror’s performance record, the Government has no reasonable expectation that the offeror will successfully perform the required effort.
Note: In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror will not be evaluated favorably or unfavorably on past performance. Therefore, the offeror shall be determined to have unknown past performance and in the context of acceptability/unacceptability, “unknown” shall be considered “acceptable.”
