About this file
This document is a Commercially Useful Function (CUF) Certification Form related to a state government contract opportunity. The form requires certified Small Business (SB), Microbusiness, and/or Disabled Veteran Business Enterprise (DVBE) contractors, subcontractors, or suppliers to certify that they meet the CUF requirements under state law when fulfilling a contract or purchase order. The form asks several questions to determine if the contractor is responsible for the execution of a distinct element of the work, performs the work themselves, and adds value through their own efforts rather than simply acting as a pass-through.
The form must be completed with an original signature by the certified business owner or officer. Failure to meet the CUF requirements could impact the contractor's ability to participate in the state government contract opportunity. No other details about the specific contract opportunity are provided in this document.
