About this file

This document is a Past Performance Information Sheet from the Executive Office of the President, designed for contractors to provide details about their previous contract work. The form requires comprehensive information about a specific contract or order, including contract identification number, description of products or services, contract status, contractor role, awarding agency, contractor identification, contract type, dollar value, performance period, and primary agency point of contact.

The form includes a section for "Description of Work / Explanations" where offerors are instructed to describe the products or services provided, explain the relevance of their performance to the current solicitation, and disclose any adverse past performance issues. The instructions emphasize the importance of accurately completing the form, noting that failure to do so may negatively impact the evaluation of the offeror's proposal or quotation. The document is a standard template for contractors to document their past performance details when bidding on federal contracts.
