Summary of Amend 5: Mobile Services Instructions and Information for Contractors
This amendment provides contractor instructions for delivering Mobile Magnetic Resonance Imaging (MRI) and Computed Tomography (CT) services at a federal institution located at 27072 Ballston Road, Sheridan, Oregon 97378. The contractor must supply both mobile units with all necessary transportation, personnel, management, and operational support. The institution will provide 480-volt, 3-phase WYE electrical hookup as required. Service access and exit operations are restricted to 7:30 a.m. to 3:00 p.m., Monday through Friday. Scheduling will be established by the Contracting Officer (CO) or Contracting Officer's Representative (COR) on a rotating basis among three facility locations (FCI, FDC, and SPC) with mutual agreement required.
Specific facility access requirements vary by location: FCI units must enter through the rear gate and park adjacent to the loading dock nearest the power source, with tractors required to disconnect and remain staged outside the secure perimeter; FDC and SPC units enter through the front gate and park at CAMP parking nearest the appropriate power source. Both mobile units must be capable of readily accommodating an approximate number of patients per session, with the specific patient volume to be determined at the discretion of Health Services and the CO. The contractor assumes full responsibility for all aspects of unit operation and service delivery.
